CT引导下血肿穿刺联合侧脑室外引流术治疗高血压性脑干出血的效果及对创伤反应的影响

Effect of CT-guided hematoma puncture combined with lateral ventricular drainage in the treatment of hypertensive brainstem hemorrhage and its influence on traumatic response

目的探讨CT引导下血肿穿刺联合侧脑室外引流术治疗高血压性脑干出血的效果及对创伤反应的影响。方法选取2020年2月至2022年2月收治的90例高血压性脑干出血患者作为研究对象,根据不同手术方法将其分为对照组与观察组,各45例。对照组采用侧脑室外引流术治疗,观察组在对照组基础上加CT引导下血肿穿刺。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。术前,两组的C反应蛋白(CRP)、皮质醇(Cor)、去甲肾上腺素(NE)、肾上腺素(E)水平比较,差异无统计学意义(P>0.05);术后,观察组的CRP、Cor、NE、E水平低于对照组,差异具有统计学意义(P<0.05)。观察组的并发症总发生率低于对照组,差异具有统计学意义(P<0.05)。结论CT引导下血肿穿刺联合侧脑室外引流术治疗高血压性脑干出血的效果满意,可减轻创伤反应,减少术后并发症。

Objective To explore the effect of CT-guided hematoma puncture combined with lateral ventricular drainage in the treatment of hypertensive brainstem hemorrhage and its influence on traumatic response.Methods A total of 90 patients with hypertensive brainstem hemorrhage admitted from February 2020 to February 2022 were selected as the research objects.According to different surgical methods,the patients were divided into control group and observation group,with 45 cases in each group.The control group was treated with lateral ventricular drainage,and the observation group was treated with CT-guided hematoma puncture on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Before operation,there were no significant differences in the levels of C-reactive protein(CRP),cortisol(Cor),norepinephrine(NE)and epinephrine(E)between the two groups(P>0.05);after operation,the levels of CRP,Cor,NE and E in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion CT-guided hematoma puncture combined with lateral ventricular drainage has satisfactory effect in the treatment of hypertensive brainstem hemorrhage,it can reduce traumatic response and postoperative complications.
