空腹血糖联合糖化血红蛋白检测在妊娠期糖尿病诊断中的临床价值

Clinical value of fasting blood glucose combined with glycated hemoglobin in diagnosis of gestational diabetesmellitus

目的探讨在妊娠期糖尿病诊断中应用空腹血糖联合糖化血红蛋白检测的临床价值。方法选取82例妊娠期糖尿病孕妇视为研究组;另选择医院同期正常产检的健康孕妇82例视为对照组。两组均进行空腹血糖、糖化血红蛋白检测,对比两组指标检测结果。结果研究组孕妇的糖化血红蛋白、空腹血糖、服糖1h后血糖、服糖2h后的血糖水平分别为(7.16±0.84)%、(7.21±0.77)mmol/L、(9.82±1.02)mmol/L、(8.52±0.98)mmol/L,均分别高于对照组的(5.89±0.62)%、(6.04±0.65)mmol/L、(6.35±0.82)mmol/L、(5.87±0.71)mmol/L(P<0.05)。联合指标检测的准确率100%,分别高于单独空腹血糖的86.59%与单独糖化血红蛋白准确率82.93%(P<0.05)。结论糖化血红蛋白联合空腹血糖检测妊娠期糖尿病的准确度更高,有利于孕妇的疾病治疗,值得临床推广。

Objective To explore the clinical value of fasting blood glucose combined with glycated hemoglobin detection in the diagnosis of gestational diabetes mellitus.Methods 82 pregnant women with gestational diabetes were selected as the study group.A total of 82 healthy pregnant women with normal birth during the same period were selected as the control group.Fasting blood glucose and glycated hemoglobin were detected in both groups,and the detection Results of the two groups were compared,and the positive rates of combined detection and single detection were compared.Results The levels of HBAlc,fasting blood glucose,blood glucose after 1 h and 2 h were(7.16±0.84)%,(7.21±0.77)mmol/L,(9.82±1.02)mmol/L and(8.52±0.98)mmol/L,respectively.It was higher than that of control group(5.89±0.62)%,(6.04±0.65)mmol/L,(6.35±0.82)mmol/L and(5.87±0.71)mmol/L,respectively(P<0.05).The accuracy of combined index detection was 100%,which was higher than 86.59%of fasting blood glucose and 82.93%of A1C alone(P<0.05).Conclusion In the clinical diagnosis of gestational diabetes mellitus,the application of HBAlc combined with fasting blood glucose detection has better accuracy,which is conducive to the treatment of pregnant women,and is worthy of clinical promotion.
