HR-HPV感染对宫颈阴道微生态、miR-21、CDK5表达的影响

摘要 目的分析高危型人乳头瘤病毒(HR-HPV)感染对宫颈阴道微生态与宫颈脱落细胞miR-21、细胞周期蛋白依赖性激酶5(CDK5)表达的影响。方法选取2020年1月—2022年12月250例宫颈炎为研究对象,根据HR-HPV感染情况分为HR-HPV阴性组(n=160)和HR-HPV阳性组(n=90)。检测HR-HPV阳性组HPV感染亚型,比较2组宫颈阴道微生态情况。采用实时荧光定量PCR法检测2组宫颈脱落细胞miR-21、CDK5表达情况。采用Spearman法分析HR-HPV感染与miR-21、CDK5表达水平和阴道微生态的关系,以及宫颈细胞miR-21、CDK5水平与宫颈阴道微生态的关系。结果HR-HPV阳性组HR-HPV感染以HPV16亚型检出率最高,占53.33%。HR-HPV阳性组阴道微生态失衡率、解脲脲原体阳性率、衣原体阳性率、乳酸杆菌异常率、细菌性阴道炎发生率均高于HR-HPV阴性组(P<0.01)。HR-HPV阳性组宫颈脱落细胞miR-21、CDK5相对表达量均高于HR-HPV阴性组(P<0.01)。宫颈脱落细胞miR-21、CDK5表达水平与细菌性阴道炎发生率、解脲脲原体阳性率、乳酸杆菌异常率、衣原体阳性率均呈正相关(P<0.01)。结论HR-HPV感染可加重宫颈阴道微生态紊乱,增加宫颈病变严重程度,使宫颈脱落细胞miR-21、CDK5表达明显升高。 Objective To analyze the effects of high-risk human papillomavirus(HR-HPV)infection on the cervicovaginal microecology and the expression of miR-21 and cyclin-dependent kinase 5(CDK5)in cervical exfoliated cells.Methods A total of 250 patients with cervicitis treated from January 2020 to December 2022 were selected as research subjects,and were divided into HR-HPV-negative group(n=160)and HR-HPV-positive group(n=90)according to HR-HPV infection.HPV infection subtypes were detected in the HR-HPV-positive group,and cervicovaginal microecology between the two groups was compared.Real-time fluorescence quantitative PCR was used to detect the expression of miR-21 and CDK5 in two groups of cervical exfoliated cells.Spearman method was used to analyze the relationship between HR-HPV infection and the expression levels of miR-21 and CDK5 and cervical microecology,and the relationship between the levels of miR-21 and CDK5 in cervical cells and cervicovaginal microecology.Results In the HR-HPV-positive group,the detection rate of HR-HPV infection was the highest with HPV16 subtype,accounting for 53.33%.The rate of vaginal microecological imbalance,positive rate of ureaplasma urealyticum(UU),positive rate of chlamydia,abnormal rate of lactobacillus and incidence of bacterial vaginitis in HR-HPV-positive group were higher than those in HR-HPV-negative group(P<0.01).The relative expression levels of miR-21 and CDK5 in cervical exfoliated cells in HR-HPV-positive group were higher than those in HR-HPV-negative group(P<0.01).The expression levels of miR-21 and CDK5 in cervical exfoliated cells were positively correlated with the incidence of bacterial vaginitis,the positive rate of UU,the abnormal rate of lactobacillus and the positive rate of chlamydia(P<0.01).Conclusion HR-HPV infection can aggravate the disturbance of cervicovaginal microecology,increase the severity of cervical lesions,and significantly increase the expression of miR-21 and CDK5 in cervical exfoliated cells.
