摘要
目的:探讨甘肃临夏地区汉族、回族和东乡族妇女人乳头瘤病毒(HPV)感染情况及基因型分布,为该地区HPV感染和宫颈癌的防治提供理论依据。方法:选取2019年10月—2020年4月临夏州人民医院就诊的1 154例妇女(汉族636例、回族276例,东乡族242例)作为研究对象,采集宫颈上皮细胞,利用PCR-反向杂交技术检测HPV的感染状况,比较不同民族间HPV感染的差异是否存在显著性。结果:临夏地区妇女HPV的阳性检出率为10.49%,回族和东乡族妇女的阳性率(分别为11.59%和12.40%)高于汉族妇女(9.30%),差异无统计学意义(P>0.05)。临夏地区感染率最高的HPV基因型依次为HPV16、HPV58、HPV52、HPV18和HPV68,并且不同民族间感染的HPV类型也略有不同。分析三个民族不同年龄段HPV感染的情况,随着年龄的增长,HPV感染率逐渐增高,在≥56岁组感染率达到最高。结论:汉族、回族和东乡族妇女HPV感染率及感染亚型存在差异,三个民族HPV感染随着患者年龄的增长都有上升的趋势,在36~45岁和46~55岁年龄段感染率为11%左右,最能代表临夏地区HPV的感染情况。
Objective:To investigate the human papillomavirus(HPV)infection and genotype distribution among Han,Hui and Dongxiang women in Linxia,Gansu,and to provide a theoretical basis for the prevention and treatment of HPV infection and cervical cancer in this region.Methods:A total of 1154 women(636 Han,276 Hui,and 242 Dongxiang)attending the hospital from October 2019 to April 2020 were selected for the study.Cervical epithelial cells were collected and tested for HPV infection status using PCR-reverse hybridization.The differences in HPV infection among different ethnic groups were compared for significance.Results:The positive detection rate of HPV among women in Linxia was 10.49%,and the positive rates of Hui and Dongxiang(11.59%and 12.40%,respectively)were higher than those of Han women(9.30%),with no statistically significant difference(P>0.05).The HPV genotypes with the highest infection rates in Linxia were,in order,HPV16,HPV58,HPV52,HPV18,and HPV68,and the types of HPVs infected vary slightly among different ethnic groups.HPV infections in the three ethnic groups were analyzed at different ages,and the prevalence of HPV infection increased progressively with age,reaching a maximum at≥56 years of age.Conclusion:There are differences in HPV infection rates and subtypes of infection among Han,Hui,and Dongxiang women,and HPV infection in all three ethnic groups tends to increase with the age of the patient,with infection rates of about 11%in the 36-45 and 46-55 year-old age groups,which is most representative of HPV infection in the Linxia area.
