从阳明经探讨颈椎病的防治

Discuss the Prevention and Treatment of Cervical Spondylosis from Yangming Meridian

颈椎病是以颈椎骨质增生、椎间盘退行性变为主导致邻近组织受累而引起一系列临床症状的总称。在当今社会,颈椎病是临床常见病、多发病,以中老年人多见,但目前颈椎病的发病率具有年轻化趋势。在中医上,颈椎病归属“痹症”范畴,其病位在颈项部,称为“项痹”,历代医家多从太阳经论治。该研究从阳明经对颈椎病进行探讨,基于关节疾病“筋骨失衡,以筋为先”的病机观,认为颈椎病的发病虽属风寒湿稽留,然而以湿为主。由于阳明胃经为多气多血之经,且脾胃居中焦,属土主肌肉,认为颈椎病的发生发展与阳明经病变关系密切,并从土能燥湿、阳明经的循行部位、多气多血的特点加以说明。从阳明经的角度论述了颈型、神经根型、椎动脉型、脊髓型、交感型、混合型六种不同分型的颈椎病的治疗,希冀为临床上颈椎病的防治提供新思路,指导中医在颈椎病的应用,从而提高临床疗效。

Cervical spondylosis is a general term for a series of clinical symptoms caused by cervical bone hyperplasia and intervertebral disc degeneration,which mainly lead to the involvement of adjacent tissues.In today’s society,cervical spondylosis is a common clinical disease and a frequently-occurring disease,which is more common in middle-aged and elderly people.However,the incidence of cervical spondylosis is currently trending younger.In traditional Chinese medicine,cervical spondylosis belongs to the category of“Bi syndrome”.The disease is located in the neck and is called“Neck Bi”.Doctors in the past dynasties mostly treated it based on the Taiyang Meridian.This study discusses cervical spondylosis from the Yangming Meridian.Based on the pathogenesis of joint diseases,“muscles and bones are imbalanced,tendons first”,it is believed that although the onset of cervical spondylosis is caused by wind,cold and dampness,it is mainly dominated by dampness.Since the Yangming Stomach Meridian is a meridian with a lot of qi and blood,and the spleen and stomach are in the middle of the body and belong to the earth-dominated muscles,it is believed that the occurrence and development of cervical spondylosis are closely related to the lesions of the Yangming Meridian,and it is based on the fact that the earth can remove dampness and the circulation of the Yangming Meridian The location and the characteristics of excessive Qi and blood will be explained.This article discusses the treatment of six different types of cervical spondylosis from the perspective of the Yangming Meridian:cervical type,nerve root type,vertebral artery type,spinal type,sympathetic type,and mixed type.It is hoped to provide new ideas for the prevention and treatment of cervical spondylosis in clinical practice.Guide the application of traditional Chinese medicine in cervical spondylosis,thereby improving clinical efficacy.
