综合干预联合协同干预在围手术期甲状腺癌患者中的应用效果

Application effect of comprehensive intervention combined with collaborative intervention in perioperative thyroid cancer patients

目的探讨综合干预联合协同干预在围手术期甲状腺癌患者中的应用效果。方法根据围手术期干预方式的不同将120例甲状腺癌患者分为常规组(n=58)和综合组(n=62),常规组患者采取常规干预,综合组患者采取综合干预联合协同干预。比较两组患者的围手术期指标、心理状态[抑郁自评量表(SDS)和焦虑自评量表(SAS)]、生活质量[欧洲癌症研究与治疗组织生命质量测定量表(EORTC QLQ-C30)]及用药依从性[Morisky用药依从性问卷(MMAS-8)]。结果综合组患者术后首次下床时间、术后固体食物耐受时间、术后住院时间均明显短于常规组(P﹤0.01)。术后第4周,两组患者SDS、SAS评分均低于本组术前,EORTC QLQ-C30各维度评分均高于本组术前,综合组患者SDS、SAS评分均低于常规组,EORTC QLQ-C30各维度评分均高于常规组,差异均有统计学意义(P﹤0.05)。术后第4周,综合组患者用药依从性明显高于常规组(P﹤0.01)。结论综合干预联合协同干预应用于围手术期甲状腺癌患者,有利于患者术后康复,可缩短患者的住院时间,改善患者的负性情绪,并提高生活质量。

Objective To investigate the application effect of comprehensive intervention combined with collabora�tive intervention in perioperative thyroid cancer patients.Method A total of 120 patients with thyroid cancer were divid�ed into conventional group(n=58,received conventional intervention)and comprehensive group(n=62,received compre�hensive intervention+collaborative intervention)according to different perioperative intervention methods.The periopera�tive indexes,psychological state[self-rating depression scale(SDS)and self-rating anxiety scale(SAS)],quality of life[European Organization for Research and Treatment of Cancer quality of life questionnaire core 30(EORTC QLQ-C30)]and medication compliance[the 8-items Morisky medication adherence scale(MMAS-8)]were compared between the two groups.Result The first time to get out of bed,the time to solid food tolerance and the postoperative hospitalization time in the comprehensive group were shorter than those in the conventional group(P<0.01).At the 4th week after sur�gery,the SDS and SAS scores were decreased and dimensional scores of EORTC QLQ-C30 were increased in both groups,with lower SDS and SAS scores and higher dimensional scores of EORTC QLQ-C30 in the comprehensive group than those in the conventional group(P<0.05).At the 4th week after surgery,the medication compliance in the compre�hensive group was higher than that in the conventional group(P<0.01).Conclusion Comprehensive intervention com�bined with collaborative intervention for perioperative thyroid cancer patients is beneficial to postoperative rehabilitation,which can shorten hospital stay,improve negative emotions and quality of life of patients.
