老年结直肠癌患者术后并发症危险因素分析

Risk factors of postoperative complications in elderly patients with colorectal cancer

目的 探讨老年结直肠癌术后并发症的危险因素。方法 回顾性分析2019年1月至2022年12月于中国科学技术大学附属第一医院(安徽省立医院)西区接受结直肠癌手术的123例患者临床资料,根据术后是否发生并发症分为有并发症组与无并发症组,分析并发症发生的危险因素。结果 38例发生并发症,并发症发生率30.89%。单因素分析显示,年龄≥75岁、术前营养不良、术前白蛋白下降、术前前白蛋白下降、小腿围下降、共病、麻醉分级Ⅲ级、手术时间>150 min均与术后并发症发生相关。二元logistic回归分析显示,年龄≥75岁、术前存在营养不良、术前白蛋白及前白蛋白较低、共病、麻醉分级Ⅲ级、手术时间>150 min均为老年结直肠癌术后并发症发生的危险因素(P<0.05)。结论 年龄≥75岁、术前存在营养不良、术前白蛋白及前白蛋白较低、共病、麻醉分级Ⅲ级及手术时间较长是老年结直肠癌患者术后并发症发生的危险因素。

Objective To explore the risk factors of postoperative complications in elderly patients with colorectal cancer.Methods The clinical data of 123 patients who underwent colorectal cancer surgery in the Western District of the First Affiliated Hospital of USTC from January 2019 to December 2022 were retrospectively analyzed,and they were divided into the complication group and the non-complication group according to whether postoperative complications occurred,and the risk factors of complications were analyzed.Results A total of 38 cases had complications,the complication rate was 30.89%.Univariate analysis showed that age≥75 years,preoperative malnutrition,preoperative albumin decline,preoperative prealbumin decline,preoperative leg circumference decline,comorbidities,gradeⅢanesthesia,and operation time>150 minutes were all associated with postoperative complications(P<0.05).Binary logistic regression analysis showed that age≥75 years old preoperative malnutrition,preoperative lower albumin and prealbumin,comorbidities,gradeⅢanesthesia,and operation time>150 minutes were all risk factors for postoperative complications of elderly colorectal cancer(P<0.05).Conclusions Age≥75 years,preoperative malnutrition,lower albumin and prealbumin,comorbidities,gradeⅢanesthesia and longer operation time are risk factors for postoperative complications in elderly patients with colorectal cancer.
