成人不同程度近视的视网膜周边离焦比较及与角膜形态参数的关系

Comparison of retinal peripheral defocus in adults with different degrees of myopia and the relationship with parameters of the cornea shape

目的比较成人不同程度近视的视网膜周边离焦(RDV),探讨其与角膜形态的关系。方法收集302例近视患者,按等效球镜(SE)分为:低度组(-3.00<SE≤-0.50D),中度组(-6.00<SE≤-3.00D),高度组(-9.00<SE≤-6.00D)。测量各区域RDV及角膜散光(AST)、表面不对称指数(SAI)等参数,比较各组RDV,并分析其与角膜形态的相关性。结果三组在偏心20°外的RDV均为远视状态,随偏心度增加而增大(P<0.05);低度组分别与中、高度组的RDV20°~53°有差异(P均<0.01);中度组与高度组的RDV20°~30°有差异(P<0.05)。TRDV与AST呈正相关(P<0.05);RDV10°~30°与SAI呈负相关(P均<0.05)。结论成人不同程度近视的视网膜周边离焦随偏心度增加而增加,在一定程度上受角膜形态的影响。

Objective To compare the retinal peripheral defocus(RDV)in adults with different degrees of myopia and to analyze the relationship between retinal peripheral defocus and shape parameters of the cornea.Methods 302 patients with myopia were collected in this study.The data were divided into 3 groups according to spherical equivalent refraction(SE):low myopia(-3.00<SE≤-0.50D),moderate myopia(-6.00D<SE≤-3.00D),and high myopia(-9.00D<SE≤-6.00D).RDV and parameters such as corneal astigmatism(AST)and surface asymmetry index(SAI)were measured in each region,and RDV was compared between the groups and analyzed their correlation with each parameter of the cornea shape.Results RDV beyond 20°of eccentricity showed hyperopia in all three groups and increased with increasing eccentricity(all P<0.05);RDV20°-53°were significantly different between the low and moderate myopia group and the low and high myopia group(all P<0.01);There was a significant difference between the moderate myopia group and the high myopia group for RDV20°-30°(P<0.05).TRDV positively correlated with AST(P<0.05);RDV10°-30°negatively correlated with SAI(all P<0.05).Conclusion Peripheral retinal defocus increases with increasing eccentricity in adults with varying degrees of myopia.To some extent,it is affected by the shape parameters of the cornea.
