脉管平滑肌刺激技术联合综合消肿疗法对妇科恶性肿瘤患者术后和(或)放疗后下肢淋巴水肿的疗效

Cellules musculaires lisses vasculaires technique combined with comprehensivce detumescene therapy for postoperative and/or post-radiotherapic lower limb lymphedema of gynecological malignant tumors

目的探讨脉管平滑肌刺激(CMLV)技术联合综合消肿疗法(CDT)对妇科恶性肿瘤患者术后和(或)放疗后下肢淋巴水肿的疗效。方法选取2020年1月至2022年6月大连市妇女儿童医疗中心(集团)等2所医院收治的妇科恶性肿瘤术后和(或)放疗后下肢淋巴水肿患者120例,随机分为研究组和对照组,每组60例。研究组采用CMLV联合CDT治疗,对照组仅采用单纯CDT治疗。比较两组患者治疗前后的淋巴水肿分期情况、双侧下肢体积差值比、妇科恶性肿瘤淋巴水肿问卷(GCLQ)评分、肢体淋巴水肿生活质量(LYMQOL)评分、总有效率等。结果研究组与对照组治疗后第0、3、6个月淋巴水肿分期下降比较差异有统计学意义(P<0.05);两组治疗后第0、3、6个月的双下肢体积差值及GCLQ评分均较治疗前下降,差异有统计学意义(P<0.001);研究组与对照组治疗后第0、3、6个月双下肢体积差值及GCLQ评分下降比较差异有统计学意义(P<0.05);两组治疗后第0、3、6个月LYMQOL评分及总体QOL评分与治疗前比较,差异均有统计学意义(P<0.05)。研究组的症状、身体形象/外观、下肢功能、情绪等较对照组改善更明显。研究组LYMQOL评分在治疗后第0、3、6个月均低于对照组,研究组总体QOL评分在治疗后第0、3、6个月均低高于对照组,差异均有统计学意义(P<0.05)。研究组的生活质量较对照组改善更为明显。结论CMLV技术联合CDT对妇科恶性肿瘤术后和(或)放疗后患者下肢淋巴水肿疗效确切,可减轻患者下肢水肿程度,提高患者生活质量,值得在临床上推广应用。

Objective To investigate the therapeutic effect of cellules musculaires lisses vasculaires(CMLV)combined with comprehensive detumescence therapy(CDT)on lymphedema of lower limbs in patients with gynecological malignant tumor after operation and/or radiotherapy.MethodsA total of 120 patients with lower limb lymphedema after operation and/or radiotherapy for gynecological malignant tumor admitted to 2 hospitals of Dalian Women and Children Medical Center(Group)from January 2020 to June 2022 were randomly divided into study group and control group,with 60 patients in each group.The study group was treated with CMLV combined with CDT,while the control group was treated with CDT alone.The stage of lymphedema,bilateral lower limb volume difference ratio,CCLQ score,LYMQOL score and total effective rate of the two groups were compared before and after treatment.Results Compared with the control group,the stage of lymphedema decreased significantly at the 0,3 and 6 months after treatment in the study group(P<0.05);the lower limb volume difference and CCLQ score of the two groups at O,3 and 6 months after treatment were significantly lower than those before treatment(P<0.001);compared with the control group,LYMQOL scores and overall QOL scores at O,3 and 6 months after treatment were significantly dfferent(P<0.05).Symptoms,body image/appearance,lower limb function and mood of the study group were improved more obviously than those of the control group.LYMQOL scores of the study group were lower than those of the control group at O,3 and 6 months after treatment,and overall QOL scores were lower than those of the control group at O,3 and 6 months after treatment,and the differences were statistically significant(P<0.05).The quality of life of the study group was improved more obviously than that of the control group.Conclusion Cellules musculaires lisses vasculaires combined with comprehensive detumescence therapy is effective for lower limb lymphedema in patients with gynecological malignant tumor after operation and/or radiotherapy,which can reduce lower limb edema and improve the quality of life of patients,and is worthy of popularization and application in clinic.
