王彦刚基于“核心病机观”从“浊毒”论治胃癌前病变

Wang Yangang's experience in treating gastric precancerous lesions from Zhuodu theory based on the core pathogenesis of diseases

王彦刚教授承古拓今提出了疾病的“核心病机观”理论,他认为浊毒内蕴脾胃为贯穿于胃癌前病变之始末的“核心病机”,浊毒胶结不解是胃癌前病变病情进展之病机关键,也是其迁延不愈、多阶段发展的重要病理因素。在治疗上,以化浊解毒为根本大法,选用自拟化浊解毒方为主要方药,随证行以清热、祛湿、行气、化积、宣郁、健脾、益胃等治法,使热邪清、浊毒化、清阳升、浊阴降,痰化瘀除,脾升胃降,气机通畅,及时阻断病情发展甚至逆转胃癌前病变,收效显著,颇具临床指导意义。

Professor Wang Yangang proposes the theory of"core pathogenesis"of diseases by inheriting ancient and exploring modern medical theories.He believes that the accumulation of turbid toxins in the spleen and stomach is the"core pathogenesis"that runs through the beginning and end of gastric precancerous lesion.The stagnation of turbid toxins is the key to the pathogenesis in the progression of gastric precancerous lesions,and is also an important pathological factor for protracted course and multi-stage development of the gastric precancerous lesion.In terms of treatment,the fundamental principle is to eliminate turbid fluid and remove toxic,and a self designed formula for eliminating turbid fluid and removing toxic is selected as the main prescription.The treatment methods of clearing heat,eliminating dampness,activating qi,relieving masses,clearing stagnation,strengthening spleen,and benefiting the stomach are carried out according to the syndrome,so as to clear heat pathogen,eliminate turbid and toxins,rise lucid yang,descende turbidity and yin,resolve phlegm and remove blood stasis,rise the spleen and lower the stomach,and ensure unobstructed qi movement.This can timely block the development of the disease and even reverse gastric precancerous lesion,with significant therapeutic effects and clinical guiding significance.
