过量补充活性维生素D致急性肾损伤1例

Acute Kidney Injury Due to Excessive Active Vitamin D Supplementation:a Case Report

目的探讨活性维生素D导致高钙血症致急性肾损伤的发病机制、临床表现及治疗转归。方法本文介绍1例严重骨质疏松伴骨折老年女性误服过量活性维生素D(骨化三醇)出现高钙血症导致急性肾损伤的病例,并查阅相关文献。结果过量补充活性维生素D易导致高钙血症,而高钙血症可导致泌尿系统、神经系统、消化系统等多系统病变。结论高钙血症临床诊断一旦确立,要立即查明病因,并采取措施迅速降低血钙,改善预后。

OBJECTIVE To explore the pathogenesis,clinical manifestation and treatment outcome of acute re⁃nal injury caused by hypercalcemia caused by active vitamin D.METHODS This article introduces a case of acute renal injury caused by hypercalcemia in an elderly woman with severe osteoporosis and fracture who mistakenly took excessive active vitamin D(calcitriol),and reviews relevant literature.RESULTS Excessive calcium supplement can easily lead to hypercalcemia,and hypercalcemia can lead to multi⁃system diseases such as urinary system,nerv⁃ous system and digestive system.CONCLUSION Once the clinical diagnosis of hypercalcemia is established,the e⁃tiology should be identified immediately,and measures should be taken to rapidly reduce blood calcium and improve the prognosis.
