某院门诊药房退药原因分析及干预措施

Analysis of Reasons for Outpatient Drug Withdrawal and Intervention Measures

目的进一步完善退药管理制度,最大限度降低退药率。方法收集某医院门诊西药房2022年6月至2022年12月的所有退药处方,按退药原因进行分类统计,并通过Excel软件进行数据处理和分析。结果半年内门诊西药房所有处方量为417152份,其中退药处方1778份,占处方总量的0.43%。产生退药的主要原因为:首先是患者因素,包括药费超过预期,慢性病患者急于看到疗效而产生抵触情绪,用药依从性差,过于担心药品的不良反应及禁忌症而拒绝服药等;其次为医师因素,包括电脑输入错误,更改患者处方等;再次为医保因素,包括医师为患者开具处方时,特殊病种项目未勾选等。结论门诊退药情况客观存在,拟通过加强退药管理标准、医务人员培训及医、药、患三者的沟通,最大限度地降低退药发生率。

OBJECTIVE Further improve the management system for drug returns and minimize the rate of drug returns.METHODS Collect all drug return prescriptions from the outpatient pharmacy of the hospital from June 2022 to December 2022,classify and analyze them according to the reasons for drug returns,and use Excel software for data processing and analysis.RESULTS Within six months,the total number of prescriptions in the outpatient Western pharmacy was 417152,of which 1778 were returned prescriptions,accounting for 0.43%of the total number of prescriptions.The primary reason for drug withdrawal is due to patient factors,including exceeding expected drug costs,chronic disease patients feeling resistant to seeing the efficacy,poor medication compliance,and excessive con⁃cerns about adverse reactions and contraindications of the drug;Secondary factors include physician factors,including computer input errors,changing patient prescriptions,etc;Generally,it is a medical insurance factor,mainly due to special disease items not selected.CONCLUSION The situation of outpatient drug withdrawal objectively exists,and it is planned to minimize the incidence of drug withdrawal by strengthening drug withdrawal management stand⁃ards,training of medical personnel,and communication among doctors,medication,and patients.
