嗜酸性粒细胞/淋巴细胞比值与慢性鼻-鼻窦炎伴鼻息肉亚分型的相关性研究

Correlation between eosinophil/lymphocyte ratio and subtypes of chroni rhinosinusitis withnasal polyps

目的探讨嗜酸性粒细胞/淋巴细胞比值(ELR)应用在慢性鼻-鼻窦炎伴鼻息肉(CRSwNP)诊断及亚分型中的价值。方法将60例CRSwNP患者作为研究对象,根据活检组织病理诊断学结果将其分为EOSCRSwNP组(n=35)和Non-EOS-CRSwNP组(n=25),收集患者临床资料并采集患者静脉血进行ELR和总IgE检测,比较两组患者各临床资料的差异性,Logistic回归分析探讨各变量与CRSwNP亚分型的相关性。结果EOS-CRSwNP组患者在总IgE、EOS百分比、EOS绝对值、ELR、变应性鼻炎伴发率和哮喘伴发率等方面明显高于Non-EOS-CRSwNP组,组间比较具有统计学意义(P<0.05)。Logistic回归分析显示:总IgE、EOS百分比、EOS绝对值和ELR与EOS-CRSwNP的发生相关(P<0.05)。结论外周血ELR水平在EOSCRSwNP患者中升高,对CRSwNP进行外周血ELR术前检测有助于组织亚分型的判定和治疗方案的制定。

Objective To investigate the value of eosinophil/lymphocyte ratio(ELR)in the diagnosis and subtype of chronic rhinosinusitis with nasal polyps(CRSwNP).Methods 60 patients with CRSwNP were divided into EOSCRSwNP group(n=35)and Non-EOS-CRSwNP group(n=25)according to the pathological diagnosis of biopsy tissue.The clinical data of patients were collected and the venous blood of patients was collected for ELR and total IgE detection,where the differences of clinical data between the two groups were compared.Logistic regression analysis was used to explore the correlation between each variable and CRSwNP subtype.Results The patients in EOSCRSwNP group were significantly higher than those in Non-EOS-CRSwNP group in total IgE,EOS percentage,EOS absolute value,ELR,allergic rhinitis concomitant rate and asthma concomitant rate,which was with statistical significance between groups(P<0.05).Logistic regression analysis showed that total IgE,EOS percentage,EOS absolute value and ELR were correlated with EOS-CRSwNP(P<0.05).Conclusions The level of ELR in peripheral blood is increased in EOS-CRSwNP patients.Preoperative detection of ELR in peripheral blood of CRSwNP is helpful to the determination of tissue subtypes and the formulation of treatment plans.
