慢性原发性耳鸣的综合治疗效果分析

Analysis of the comprehensive treatment effect of chronic primary tinnitus

目的探讨音乐疗法结合耳鸣咨询、认知行为疗法对慢性原发性耳鸣的治疗效果。方法招募70名慢性原发性耳鸣患者作为研究对象,研究组(40人)采取音乐疗法联合耳鸣咨询、认知行为疗法,对照组(30人)采取耳鸣咨询联合认知行为疗法,分别记录两组患者治疗前,治疗后3月、6月、12月的THI量表及VAS量表评测结果,进行统计学分析,对比两组患者治疗效果。结果实验组THI评分治疗前平均值36.70±20.166,治疗后12月评分平均值22.05±16.699,对照组THI评分治疗前平均值39.20±19.983,治疗后12月评分平均值30.13±17.051。实验组VAS评分治疗前平均值5.85±1.916,治疗后12月评分平均值2.05±0.914,对照组VAS评分治疗前平均值5.73±1.964,治疗后12月评分平均值4.27±1.893。通过重复测量方差分析,评估两组患者随治疗时间的变化对其THI、VAS评分的影响,结果分别为F(1.507,102.494)=9.618,P=0.001;F(2.256,153.426)=41.443,P<0.001,治疗方法、时间对THI、VAS评分的影响均有统计学意义,且治疗方法与治疗时间之间存在交互作用。结论联合采用音乐疗法、耳鸣咨询、认知行为疗法可以改善慢性原发性耳鸣患者THI、VAS评分,综合治疗是慢性原发性耳鸣的有效治疗方法。

Objective To investigate the therapeutic effect of music therapy combined with tinnitus counseling and cognitive behavior therapy on chronic primary tinnitus.Method To recruit 70 patients with chronic primary tinnitus as a research object,research group(40)take music therapy combined tinnitus counseling,cognitive behavior therapy,control group(30)take tinnitus counseling combined cognitive behavior therapy,record two groups before treatment,treatment after March,June,December THI scale and VAS scale evaluation results,statistical analysis,compare the treatment effect of the two groups of patients.Results The pre-treatment THI score was 36.70±20.166,22.05±16.699,the control THI score was 39.20±19.983,and 30.13±17.051.The VAS score was 5.85±1.916,the 2 months 2.05±0.914 after treatment,the VAS score 5.73±1.964 and 4.2 months 1.27±1.893.A repeated measures ANOVA was used to assess the effect of changes with treatment time on their THI and VAS scores,resulting in F(1.507,102,102.494)=9.618,P=0.001;F(2.256,153.426)=41.443,P<0.001,the effect of THI and VAS scores,and an interaction between treatment and treatment time.Conclusion Combining music therapy,tinnitus counseling and cognitive behavior therapy can improve THI and VAS scores in patients with chronic primary tinnitus,and comprehensive treatment is an effective treatment for chronic primary tinnitus.
