摘要
目的:探讨火针结合髋膝训练治疗膝骨性关节炎的临床疗效。方法:随机将60例膝骨性关节炎患者分为观察组与对照组。观察组给予火针结合髋膝训练治疗,对照组给予火针治疗,两组均以治疗10次为1个疗程,治疗两个疗程,共40 d。于治疗后第3个月进行随访,以疼痛视觉模拟量表(VAS评分)、安大略西部麦克马斯特大学骨关节炎指数(WOMAC评分)、关节活动度(ROM)及血清肿瘤坏死因子-α(TNF-α)及白介素-1β(IL-1β)水平作为疗效标准。结果:治疗前两组一般资料、VAS评分、WOMAC评分、关节活动度及血清TNF-α和IL-1β水平比较,差异均无统计学意义(P>0.05)。治疗后,两组患者VAS评分、WOMAC评分、关节活动度及血清TNF-α和IL-1β水平均优于治疗前,差异有统计学意义(P<0.05),且观察组优于对照组,差异有统计学意义(P<0.01)。结论:火针结合髋膝训练治疗能够缓解KOA患者疼痛、僵硬症状,改善关节功能及膝关节活动度,降低血清TNF-α、IL-1β水平。
Objective:To observe the clinical effect of fire needling combined with hip-knee training in treatment of knee osteoarthritis(KOA).Methods:60 patients with KOA were randomly divided into the observation group and the control group.The control group was treated with fire needling;on which basis,the observation group was also treated with hip-knee training.10 times of treatment were taken as one course of treatment,both groups were treated for 2 courses of 40 days.The patients were followed up 3 months after the treatment.The pain visual analogue scale(VAS)score,Western Ontario McMaster University osteoarthritis index(WOMAC)score,range of motion(ROM)and serum levels of TNF-αand IL-1βwere used as efficacy criteria.Results:There were no significant differences in general data,VAS score,WOMAC score,ROM and serum levels of TNF-αand IL-1βbetween the two groups before the treatment(P>0.05).After the treatment,VAS score,WOMAC score,ROM and serum levels of TNF-αand IL-1βwere improved compared with those before the treatment in the two groups(P<0.05),of which the improvements were more significant in the observation group than those in the control group(P<0.01).Conclusion:Fire needling combined with hip-knee training can relieve pain and stiffness symptoms in KOA patients,it can improve joint function and knee joint mobility,and reduce serum levels of TNF-αand IL-1β.
