山东地区某医院脑脊液培养阳性肺炎克雷伯菌的临床分布和耐药性分析

Clinical Distribution and Antibiotic Resistance of Klebsiella pneumoniae for Cerebrospinal Fluid Infection in a tertiary hospital in Shandong

摘要 目的:分析山东第一医科大学第一附属医院肺炎克雷伯菌脑脊液分离株的临床分布并进一步探索这些菌株的耐药特征。方法:收集2018-2022年山东第一医科大学第一附属医院脑脊液培养阳性的肺炎克雷伯菌菌株,采用K-B纸片法进行药敏实验。结果:共分离得到脑脊液培养阳性的肺炎克雷伯菌菌株35株,检出数最多的科室为神经外科。41~60岁的中年患者以及男性患者是分离率最高的人群。除阿米卡星外,肺炎克雷伯菌对其余抗生素的耐药率均在50%以上。结论:神经外科、男性及41~60岁的中年患者应该成为重点关注人群。由于耐碳青霉烯类肺炎克雷伯菌在这些菌株中占据较大比例。动态掌握脑脊液培养阳性肺炎克雷伯菌的临床分布和耐药特征,对于指导临床抗生素的合理使用和院感防控至关重要。 Objective:To analyze the distribution of Klebsiella pneumoniae(K.pneumoniae)isolated from cerebrospinal fluid(CSF)in the First Affiliated Hospital of Shandong First Medical University and further explore the antibiotic resistance characteristics.Methods:CSF K.pneumoniae strains were isolated from 2018 to 2022 in the First Affiliated Hospital of Shandong First Medical University.Antimicrobial susceptibility tests were carried out by using agar dilution method.Results:35 K.pneumoniae strains were isolated.Neurosurgery ward,male and middle-aged patients were the main population.Except amikacin,resistance rates for antibiotics tested in this study were all above 50%.Conclusion:Midle-aged,male and the patients in neurosurgery ward should be paied to more attention.Considering the high detection rate of carbapenem-resistant K.pneumoniae in CSF in this hospital,dynamic analyzing the distribution and the antibiotic resistance characteristics of K.pneumoniae isolated from cerebrospinal fluid is very important for guiding the rational use of antibiotics and the prevention and control of hospital infection.
