小榄镇区非孕期及孕期女性甲状腺激素参考范围的建立

摘要 目的:建立小榄镇区适龄女性从非孕期到产前四个阶段:非孕期(T0)、孕早期(T1)、孕中期(T2)、孕晚期(T3)特异性甲状腺激素参考范围,并以此为本地区女性甲状腺功能异常诊断提供可靠的依据。方法:纳入2022年1月-12月在我院进行备孕、健康体检以及产检的女性为研究对象,其中T0期126例,T1期2740例,T2期2600例,T3期400例,采用电化学发光法分析法测定血清促甲状腺激素(TSH)、游离甲状腺素(FT4),甲状腺过氧化物酶抗体(TPOAb)的水平,采用双侧限值(p2.5~p97.5)建立非孕期及孕早、中、晚期特异性的TSH、FT4的参考范围。结果:①TO、T1、T2和T3期的TSH参考范围分别为:0.21~5.64mIU/L,0.06~4.1mIU/L,0.278~4.90mIU/L,0.29~5.74mIU/L;FT4的参考范围是:11.42~21.98 pmol/L,12.92~22.18 pmol/L,11.47~20.72 pmol/L,10.08~21.44pmol/L。②孕早、中、晚三期TSH与FT4均呈负相关。结论:不同孕期甲状腺功能指标参考值不同,且随孕周增加而升高,因此建立本地区非孕期及孕期甲状腺功能指标参考值有利于备孕及孕期甲状腺疾病的筛查及诊治。
