沙库巴曲缬沙坦联合曲美他嗪对心肌梗死PCI术后患者心室重构指标的影响

Effect of sacubitril valsartan combined with trimetazidine on ventricular remodeling indexes in patients with myocardial infarction after PCI

目的探究沙库巴曲缬沙坦联合曲美他嗪治疗心肌梗死经皮冠状动脉介入治疗(PCI)术后的效果。方法择取2019年5月至2021年12月我院收治的108例心肌梗死PCI术后患者为研究对象,根据术后治疗方案的不同将其分为对照组和观察组,各54例。对照组PCI术后当天接受曲美他嗪治疗,观察组在对照组基础上联合沙库巴曲缬沙坦治疗。比较两组的治疗效果。结果治疗后,观察组的室间隔厚度(IVST)、左室质量指数(LVMI)、左心室质量(LVM)低于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的心肌梗死溶栓试验分级(TIMI)、TIMI心肌灌注分级(TMPG)分级中Ⅱ级占比低于对照组,Ⅲ级占比高于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的心输出量(CO)、每搏心输出量(SV)、左心室射血分数(LVEF)、舒张早期最大峰值流速/舒张晚期最大峰值流速(E/A)高于对照组,差异具有统计学意义(P<0.05)。结论沙库巴曲缬沙坦联合曲美他嗪治疗心肌梗死PCI术后患者的效果较好,可改善心肌灌注状况,缓解心室重构,促进心功能恢复。

Objective To explore the effect of sacubitril valsartan combined with trimetazidine in the treatment of myocardial infarction after percutaneous coronary intervention(PCI).Methods A total of 108 patients with myocardial infarction after PCI admitted in our hospital from May 2019 to December 2021 were selected as the research objects.According to the different postoperative treatment plan,the patients were divided into control group and observation group,with 54 cases in each group.The control group was treated with trimetazidine on the day after PCI,and the observation group was treated with sacubitril valsartan on the basis of the control group.The therapeutic effects of the two groups were compared.Results After treatment,the interventricular septal thickness(IVST),left ventricular mass index(LVMI)and left ventricular mass(LVM)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the proportion of gradeⅡin thrombolysis in myocardial infarction(TIMI)and TIMI myocardial perfusion grade(TMPG)in the observation group were lower than those in the control group,and the proportion of gradeⅢwere higher than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the cardiac output(CO),stroke volume(SV),left ventricular ejection fraction(LVEF),and early diastolic maximum peak velocity/late diastolic maximum peak velocity(E/A)of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The sacubitril valsartan combined with trimetazidine in the treatment of patients with myocardial infarction after PCI has a better effect,which can improve myocardial perfusion,relieve ventricular remodeling and promote the recovery of cardiac function.
