急性心肌梗死患者血清MYO、cTnI、FIB的表达水平及临床意义

摘要 目的探究肌红蛋白(MYO)、心肌肌钙蛋白I(cTnI)、纤维蛋白原(FIB)在急性心肌梗死患者血清中的表达水平及临床意义。方法收集河南科技大学第二附属医院2021年3月至2023年3月收治的86例急性心肌梗死患者临床资料作为观察组,另外收集同时期入院检查的56例非急性心肌梗死者临床资料作为对照组,比较两组MYO、cTnⅠ、FIB水平,以及三者对急性心肌梗死的预测价值。结果两组年龄、性别、体质量指数、吸烟、饮酒、高血压、冠心病、糖尿病、高血脂差异无统计学意义(P>0.05)。观察组MYO、cTnⅠ、FIB水平均高于对照组,差异有统计学意义(P<0.05)。MYO、cTnⅠ、FIB表达水平随冠状动脉狭窄程度的加重而增加,不同冠状动脉狭窄程度的MYO、cTnI、FIB水平比较,差异有统计学意义(P<0.05)。MYO、cTnⅠ、FIB以及三者联合的AUC面积分别为0.995、0.988、0.977、0.999,三者联合的AUC面积最大,灵敏度相对单一指标较高,预测效能相对较好。结论MYO、cTnⅠ、FIB三者联合对急性心肌梗死有良好的预测价值,可作为临床诊断急性心肌梗死的辅助工具。 Objective To investigate the expression levels and clinical significance of myoglobin(MYO),cardiac troponin I(cTnI),and fibrinogen(FIB)in the serum of patients with acute myocardial infarction.Methods Clinical data of 86 patients with acute myocardial infarction admitted to the Second Affiliated Hospital of Henan University of Science and Technology from March 2021 to March 2023 were collected as the observation group.In addition,clinical data of 56 non acute myocardial infarction patients admitted for examination during the same period were collected as the control group.MYO,cTnⅠ,FIB levels and their predictive value for acute myocardial infarction were compared between the two groups.Results There was no significant difference in age,sex,body mass index,smoking,drinking,hypertension,coronary heart disease,diabetes and hyperlipidemia between the two groups(P>0.05).The levels of MYO,cTnⅠ,and FIB in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The expression levels of MYO,cTnI,and FIB increase with the severity of coronary artery stenosis.The differences in MYO,cTnI,and FIB levels among different degrees of coronary artery stenosis are statistically significant(P<0.05).The AUC areas of MYO,cTnⅠ,FIB,and their combination are 0.995,0.988,0.977,and 0.999,respectively.The combined AUC area of MYO,cTnⅠ,FIB,and their combination is the largest,with relatively high sensitivity and predictive performance compared to a single indicator.Conclusion The combination of MYO,cTnⅠ,and FIB has good predictive value for acute myocardial infarction and can be used as an auxiliary tool for clinical diagnosis of acute myocardial infarction.
