MRI与CT诊断脑梗死并脑出血的应用价值

Comparative observation on the application value and accuracy of MRI and CT in diagnosing cerebral infarction with cerebral haemorrhage

摘要 目的:对比脑梗死并脑出血诊断中磁共振成像(MRI)与计算机断层成像(CT)的应用价值。方法:选择2022年1月—2023年4月阳江市人民医院收治的76例疑似脑梗死并脑出血患者作为研究对象。对患者开展MRI检查与CT检查,以脑血管造影结果为金标准,比较MRI与CT检查诊断效能及不同部位、不同直径病灶检出准确率。结果:脑血管造影检出阳性74例,阴性2例;MRI检出阳性74例,阴性2例;CT检查检出阳性64例,阴性12例;MRI诊断准确率、灵敏度分别为97.37%、98.65%,均高于CT检查的81.58%、83.78%(P<0.05);MRI、CT诊断特异度比较差异无统计学意义(P>0.05);MRI对脑叶、基底节病灶的诊断准确率高于CT(P<0.05);MRI检查直径≤2 mm病灶的准确率高于CT(P<0.05);病灶直径>2 mm时MRI、CT检出准确率差异无统计学意义(P>0.05)。结论:脑梗死并脑出血诊断应用MRI与CT均有较高价值,MRI对不同部位、不同直径病灶检出准确率更高,诊断方面存在明显优势。 Objective To compare the value of magnetic resonance imaging(MRI)and computed tomography(CT)in the diagnosis of cerebral infarction complicated with cerebral hemorrhage.Methods A total of 76 patients with suspected cerebral infarction and cerebral hemorrhage admitted to Yangjiang People's Hospital from January 2022 to April 2023 were selected as the study objects.MRI and CT were performed in the patients.The results of cerebral angiography were used as the gold standard to compare the diagnostic efficiency of MRI and CT and the detection accuracy of lesions of different locations and diameters.Results Cerebral angiography was positive in 74 cases and negative in 2 cases.MRI was positive in 74 cases and negative in 2 cases.CT examination detected 64 positive cases,12 negative cases;The diagnostic accuracy and sensitivity of MRI were 97.37%and 98.65%,respectively,higher than those of CT examination 81.58%and 83.78%(P<0.05).There was no significant difference in diagnostic specificity between MRI and CT(P>0.05).The diagnostic accuracy of MRI was higher than that of CT(P<0.05).When the lesion diameter was less than 2 mm,the detection accuracy of MRI was higher than that of CT(P<0.05).There was no difference in detection accuracy between MRI and CT when the lesion diameter was>2 mm(P>0.05).Conclusion Both MRI and CT are of high value in the diagnosis of cerebral infarction complicated with cerebral hemorrhage.MRI has a higher detection accuracy for lesions of different locations and diameters,and has obvious advantages in diagnosis.
