表面肌电指导治疗中风恢复期吞咽困难临床观察

Superficial Electromyography⁃guided in the Treatment of Dysphagia during Convalescence of Apoplexy

目的探索与传统治疗相比,基于表面肌电指导选穴治疗中风恢复期吞咽困难的临床效果观察。方法选取2021年1月—2022年12月成都市青羊区中医医院、成都市第一骨科医院、青羊区第九人民医院收治的共80例中风恢复期且有吞咽困难患者,按照随机数字表法分为对照组及肌电组,各40例。比较2组治疗前、治疗后及1个月随访期患者洼田饮水、反复唾液吞咽试验、吞咽功能评估量表的变化情况。结果与治疗前比较,2组治疗后洼田饮水、反复唾液吞咽试验、吞咽功能评估量表评分均显著改善(P<0.05);与对照组比较,肌电组1个月后随访在洼田饮水等级及吞咽功能评估量表评分改善更明显(P<0.05)。结论联合治疗中风恢复期吞咽困难有明显疗效,基于表面肌电指导针刺选穴在治疗后即刻及1个月随访中疗效更佳。

Objective To explore the clinical effect of superficial electromyography⁃guided acupoint selection in the treatment of dysphagia in the recovery period of stroke compared with traditional acupuncture therapy.Methods From January 2021 to December 2022,a total of 80 patients with dysphagia were enrolled in Chengdu Qingyang District Hospital of Traditional Chinese Medicine,Chengdu First Orthopedic Hospital and Qingyang District Ninth People's Hospital,and they were divided into control group and electromyography group according to the random number table,with 40 cases in each group.The changes of Watian drinking water,repeated saliva swallowing test and swallowing function assessment scale were compared between the two groups before treatment,after treatment and during the one⁃month follow⁃up period.Results Compared with the control group,the scores of water drinking,repeated saliva swallowing test and swallowing function assessment scale were significantly improved in the control group and the electromyography group after treatment(P<0.05),and compared with the control group,the electromyography group had a more significant improvement in the drinking water grade and swallowing function assessment scale score in Watian after one month(P<0.05).Conclusion The combination of acupuncture has obvious efficacy in the treatment of dysphagia in the recovery period of stroke,and the acupuncture point selection based on superficial electromyography⁃guided acupoint selection has a better effect immediately after treatment and in the follow⁃up of 1 month.
