养血补肾调冲方联合雌孕激素序贯疗法治疗肾虚血瘀型卵巢早衰临床研究

Clinical Study on Yangxue Bushen Tiaochong Prescription Combined with Estrogen-Progesterone Sequential Therapy for Premature Ovarian Failure of Kidney Deficiency and Blood Stasis Type

目的:观察养血补肾调冲方联合雌孕激素序贯疗法治疗肾虚血瘀型卵巢早衰的临床疗效。方法:采用随机数字表法将96例肾虚血瘀型卵巢早衰患者分为对照组与治疗组各48例。对照组给予雌孕激素序贯疗法治疗,治疗组在对照组基础上联合养血补肾调冲方治疗,2组连续治疗1个月。比较2组治疗前后中医证候评分、性激素指标及子宫、卵巢超声检测指标,评估2组临床疗效及不良反应发生情况。结果:治疗组总有效率95.83%,高于对照组81.25%(P<0.05)。2组治疗后中医证候评分及促卵泡生成素(FSH)、促黄体生成素(LH)水平均较治疗前降低(P<0.05),雌二醇(E_(2))及子宫三径线之和、子宫内膜厚度、卵巢平均体积水平升高(P<0.05),且治疗组治疗后子宫三径线之和、子宫内膜厚度、卵巢平均体积及E_(2)水平均高于对照组(P<0.05),中医证候评分及FSH、LH水平均低于对照组(P<0.05)。治疗组不良反应发生率2.08%,低于对照组14.58%(P<0.05)。结论:养血补肾调冲方联合雌孕激素序贯疗法治疗肾虚血瘀型卵巢早衰疗效确切,能够调节患者血清性激素水平,改善子宫及卵巢萎缩,安全性较高。

Objective:To observe the clinical effect of Yangxue Bushen Tiaochong Prescription combined with estrogen-progesterone sequential therapy for premature ovarian failure of kidney deficiency and blood stasis type.Methods:A total of 96 patients with premature ovarian failure of kidney deficiency and blood stasis type were divided into the control group and the treatment group according to the random number table method,with 48 cases in each group.The control group was treated with estrogenprogesterone sequential therapy,and the treatment group was additionally treated with Yangxue Bushen Tiaochong Prescription based on the treatment of the control group.Both groups were treated continuously for one month.The traditional Chinese medicine syndrome(TCM)scores,sex hormone indicators,and uterine and ovarian ultrasound detection indicators were compared before and after treatment in the two groups.The clinical effects and incidence of adverse reactions were evaluated in the two groups.Results:The total effective rate was 95.83%in the treatment group,higher than that of 81.25%in the control group(P<0.05).After treatment,the TCM syndrome scores and levels of follicle stimulating hormone(FSH)and luteinizing hormone(LH)in the two groups were decreased when compared with those before treatment(P<0.05),and estradiol(E_(2))and the sum of three diameters of the uterus,endometrial thickness,and average ovarian volume levels were increased(P<0.05);after treatment,the sum of three diameters in uterus,endometrial thickness,average ovarian volume level and E_(2) level in the treatment group were higher than those in the control group(P<0.05),and the TCM syndrome scores,and the levels of FSH and LH were lower than those in the control group(P<0.05).The incidence of adverse reactions was 2.08%in the treatment group,lower than that of 14.58%in the control group(P<0.05).Conclusion:The combination of Yangxue Bushen Tiaochong Prescription combined with estrogen-progesterone sequential therapy has a definite curative effect on premature ovarian failure of kidney deficiency and blood stasis type,which can regulate the sex hormone levels in serum,improve uterine and ovarian atrophy,and have high safety.
