穴位按摩、口舌操联合冰棒冷刺激对脑卒中后吞咽障碍的康复护理研究

A Study on Rehabilitation Nursing of Poststroke Swallowing Disorders by Acupoint Massage,Mouth and Tongue Exercise Combined with Popsicle Cold Stimulation

目的探讨脑卒中吞咽障碍患者实施穴位按摩、口舌操联合冰棒冷刺激的护理效果与价值。方法选择2021年1月至2022年12月我院收诊治疗脑卒中吞咽障碍患者90例进行分析研究,根据随机编号抽签方式进行分组,对照组(45例)患者予以口舌操、冰棒冷刺激,观察组(45例)在对照组的基础上联合穴位按摩,比较两组康复效果。结果经饮水试验评测结果显示,观察组等级优于对照组,差异存在统计学意义(P<0.05)。干预后,观察组吞咽功能障碍评分高于对照组,差异存在统计学意义(P<0.05)。干预前,两组生存质量相关指标比较差异不明显,P>0.05;干预后,观察组患者护理后生存质量相应分项评估得分高于对照组,差异存在统计学意义(P<0.05)。观察组出现1例吸入性肺炎,发生率为2.22%,对照组发生7例吸入性肺炎,发生率为15.56%,比较差异χ^(2)=4.939,P<0.05。结论脑卒中吞咽障碍患者通过采用穴位按摩联合口舌操、冰棒冷刺激能够有效降低患者吞咽障碍,提高患者生存质量,降低吸入性肺炎的发生率。

Objective To explore the nursing effect and value of acupoint massage,oral and tongue exercises,combined with ice cream cold stimulation on stroke patients with swallowing disorders.Methods Ninety patients diagnosed with stroke and with dysphagia who underwent treatment in our hospital from January 2021 to December 2022 were selected by drawing lots based on their numbers and grouped according to the results.The control group(45 cases)received oral and tongue exercises and ice cream cold stimulation,while the observation group(45 cases)received acupoint massage on the basis of the control group to compare the rehabilitation effects of the two groups.Results The evaluation results of the drinking water test showed that the observed patient's relevant level was better than that of the control patient,and the difference was significant between the groups(P<0.05).After corresponding intervention,the swallowing dysfunction score evaluated by the patients was higher than that of the control patients,and the difference between the groups was significant(P<0.05).Before intervention,there was no significant difference in quality of life related indicators between the two groups(P>0.05);After intervention,the corresponding sub evaluation scores of quality of life in the observation group were higher than those in the control group after nursing care,with a statistically significant difference(P<0.05).The observation group had 1 case of aspiration pneumonia,with an incidence rate of 2.22%,while the control group had 7 cases of aspiration pneumonia,with an incidence rate of 15.56%.The comparative difference wasχ^(2)=4.939,P<0.05.Conclusions Patients with swallowing disorders after stroke can effectively reduce their swallowing disorders,improve their quality of life,and reduce the incidence of aspiration pneumonia by using acupoint massage combined with oral and tongue exercises,and cold stimulation with popsicles.
