统筹常规与非常规:农村脱贫人口医疗保障政策调整影响与优化方向

Combination of Regular and Irregular Management:The Influence and Optimization Direction of Medical Security Policy Adjustment for Rural Population out of Poverty

农村医疗保障问题一直伴随着我国脱贫攻坚与乡村振兴的全过程,农村医疗保障政策关涉亿万农民群众的健康需求,其政策转型调试不仅要适应过渡期的政策变化,更应着眼于全面建成共同富裕的社会主义现代化强国进行顶层设计、统筹谋划。目前,虽然贫困人口基本医疗有保障目标实现,但因病致贫、因病返贫风险仍然存在,脱贫地区农村卫生和医疗资源仍处于劣势地位,不断提升农村医疗保障和医疗资源的公平性、可及性、充分性与可承受性仍然是政策调整过程中需关注的重要论题。本文将文本政策分析与实地调研相结合,从公共管理视角分析脱贫人口医疗保障政策调整影响以及政策调整过程中的管理问题,在此基础上提出更新医疗管理理念与方式,缩小待遇差距,强化疾病管理等统筹非常规与常规的医疗保障政策的优化方向,进一步丰富医疗管理理论与实践经验,发挥统筹非常规与常规医疗政策的功能与优势,实现农村医疗保障政策转型与平稳过渡,促进医疗保障的现代化发展。

The problem of rural medical security has always been accompanied by the whole process of poverty alleviation and rural revitalization in China. Rural medical security policy is related to the health needs of hundreds of millions of farmers. Its policy transformation and debugging should not only adapt to the policy changes in the transition period,but also focus on building a socialist modern power with common prosperity to carry out toplevel design and overall planning. At present,although the basic medical security goal of the poor population has been achieved,the risk of poverty caused by illness and returning to poverty due to illness still exists. Rural health and medical resources in poverty-stricken areas are still at a disadvantage. Continuously improving the fairness,accessibility,adequacy and affordability of rural medical security and medical resources are still important topics that need more attention in the process of policy adjustment. This paper combines text policy analysis with field research to analyze the impact of medical security policy adjustment of the poverty-stricken population and the management problems in the process of policy adjustment from the perspective of public management. On this basis,this paper proposes to update the concept and method of medical management,narrow the treatment gap,strengthen disease management and other optimization paths for coordinating Regular and Irregular medical security policies,further enrich the theory and practical experience of medical management,give full play to the functions and advantages of coordinating unconventional and conventional medical policies,realize the transformation and smooth transition of rural medical security policies,and promote the modernization of medical security development.
