固冲止崩汤联合地屈孕酮治疗围绝经期异常子宫出血的临床疗效

Clinical effect of guchong zhibeng decoction combined with dydrogesterone in the treatment of abnormal uterine bleeding in perimenopausal period

目的 探究围绝经期异常子宫出血患者采用固冲止崩汤联合地屈孕酮治疗的临床效果。方法 选取70例围绝经期异常子宫出血患者,根据治疗方法不同分为两组,每组35例。对照组单用地屈孕酮治疗,试验组在对照组基础上联合固冲止崩汤治疗。比较两组患者性激素水平、症状消失时间、中医证候积分及不良反应发生情况。结果 治疗后试验组患者促黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)及孕酮(P)水平均明显低于对照组(P<0.05)。试验组患者出血控制时间及出血症状完全消失时间均明显短于对照组(P<0.05)。治疗后试验组患者经血色淡或有块、小腹疼痛、面色萎黄及神疲气短积分均明显低于对照组(P<0.05)。试验组患者的不良反应发生率5.71%显著低于对照组的22.86%(P<0.05)。结论 固冲止崩汤联合地屈孕酮治疗围绝经期异常子宫出血的临床疗效确切,不仅可以调节性激素水平,还能够更快控制临床症状,具有安全性,值得临床推广。

Objective To explore the clinical effect of guchong zhibeng decoction combined with dydrogesterone in the treatment of perimenopausal patients with abnormal uterine bleeding.Methods 70 patients with abnormal uterine bleeding in perimenopausal period were selected and divided into two groups according to different treatment methods,with 35 cases in each group.The control group was treated with dydrogesterone alone,and the experimental group was treated with guchong zhibeng decoction on the basis of the control group.Sex hormone levels,symptom disappearance time,TCM syndrome score and adverse reactions were compared between the two groups.Results After treatment,the levels of luteinizing hormone(LH),follicle-stimulating hormone(FSH),estradiol(E_(2))and progesterone(P)in experimental group were significantly lower than those in control group(P<0.05).The time of bleeding control and complete disappearance of bleeding symptoms in experimental group were significantly shorter than those in control group(P<0.05).After treatment,the scores of pale or lumpy blood,pain in lower abdomen,pale complexion and short breath in experimental group were significantly lower than those in control group(P<0.05).The incidence of adverse reactions in experimental group(5.71%)was significantly lower than that in control group(22.86%)(P<0.05).Conclusion Guchong zhibeng decoction combined with dydrogesterone has a definite clinical effect in the treatment of abnormal uterine bleeding in perimenopause.It can not only regulate the level of sex hormones,but also control clinical symptoms faster.It is safe and worthy of clinical promotion.
