HPV联合TCT检测在宫颈癌筛查中的价值分析

Value analysis of HPV combined with TCT in cervical cancer screening

目的评估人乳头瘤病毒(HPV)联合液基薄层细胞学检查(TCT)在宫颈癌筛查中的价值。方法选取本院1251例实施的宫颈活检术病例,均接受HPV、TCT、病理学检查。根据HPV及TCT的结果分为HPV阳性+TCT阳性组(589例)、HPV阳性+TCT阴性组(367例)HPV阴性+TCT阳性组(295例)。比较三组与宫颈活检病理检查的检出率以及宫颈低级别病变与宫颈高级别病变的占比情况。结果HPV阳性+TCT阳性组的宫颈病变检出率(98.47%)高于HPV阳性+TCT阴性组(68.39%)、HPV阴性+TCT阳性组(34.92%),HPV阳性+TCT阴性组的宫颈病变检出率高于HPV阴性+TCT阳性组,差异有统计学意义(P<0.05)。HPV阳性+TCT阳性组的宫颈高级别病变检出率最高(82.59%),其次为HPV阴性+TCT阳性组(78.64%)、HPV阳性+TCT阴性组(24.70%),两两比较差异有统计学意义(P<0.05)。结论HPV联合TCT检测对宫颈病变的筛查检出率最高;对于宫颈低级别病变,可能HPV筛查效率更高,TCT筛查效率较低;但对于宫颈高级别病变,HPV联合TCT检查对于宫颈高级别病变筛查率最高。

Objective To evaluate the value of human papillomavirus(HPV)combined with liquid-based thin layer cytology(TCT)in cervical cancer screening.Methods 1251 cases of cervical biopsy performed in our hospital were selected,and all of them received HPV,TCT and pathology examination.According to the results of HPV and TCT,they were divided into HPV positive+TCT positive group(589 cases),HPV positive+TCT negative group(367 cases),and HPV negative+TCT positive group(295 cases).The detection rate of cervical biopsy and pathological examination and the proportion of low-grade cervical lesions and high-grade cervical lesions were compared between the three groups.Results The detection rate of cervical lesions in HPVpositive+TCT positive group(98.47%)was higher than that in HPV-positive+TCT negative group(68.39%)and HPV-negative+TCT positive group(34.92%),and the detection rate of cervical lesions in HPV-positive+TCT negative group was higher than that in HPV-negative+TCT positive group.The difference was statistically significant(P<0.05).The detection rate of high-grade cervical lesions in HPV-positive+TCT positive group was the highest(82.59%),followed by HPV-negative+TCT positive group(78.64%)and HPV-positive+TCT negative group(24.70%),with statistical significance(P<0.05).Conclusion HPV combined with TCT had the highest detection rate for cervical lesions.For low-grade cervical lesions,HPV screening efficiency may be higher,TCT screening efficiency is lower;However,for high-grade cervical lesions,HPV combined with TCT has the highest screening rate for highgradecervicallesions.
