益肾壮骨汤配合三步推拿点穴法治疗膝骨关节炎的效果及对膝关节功能的影响

Effect of Yishen Zhuanggu Decoction combined with three-step massage and acupoint on knee joint function inpatients with knee osteoarthritis

目的 观察益肾壮骨汤配合三步推拿点穴法治疗膝骨关节炎的疗效及对膝关节功能的影响。方法 将84例膝骨关节炎患者按照随机数字表法分为2组,对照组42例予益肾壮骨汤治疗,治疗组42例在对照组基础上配合三步推拿点穴法治疗。2组均治疗4周。比较2组治疗前后日常生活能力评分(ADL)、膝关节功能评估量表(KSS)评分、疼痛视觉模拟量表评分(VAS),膝周压痛、关节活动度、关节肿胀评分及炎症因子水平,并统计2组疗效。结果 治疗组总有效率97.62%(41/42),对照组总有效率83.33%(35/42),治疗组临床疗效优于对照组(P<0.05)。2组治疗后ADL、KSS评分均较本组治疗前升高(P<0.05),且治疗组治疗后均高于对照组(P<0.05),2组治疗后疼痛VAS均降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。2组治疗后各项症状评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后血清IL-6、TNF-α、IL-1水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。结论 益肾壮骨汤配合三步推拿点穴法治疗膝骨关节炎效果明显,患者日常生活能力、膝关节功能明显提升,疼痛感减轻,炎症因子水平下降,值得临床推广。

Objective To explore the effect of Yishen Zhuanggu Decoction combined with three-step massage and acupoint on knee joint function in patients with knee osteoarthritis(KOA)and its influence.Methods A total of 84 KOA patients were randomly assigned into the treatment group(n=42)and the control group(n=42).All patients were managed by Yishen Zhuanggu Decoction,and those in the treatment group were additionally treated with three-step massage and acupoint.Treated for 4 weeks,the aim was to compare Activities of Daily Living(ADL),Knee Society Score(KSS),Visual Analogue Scale(VAS)for pain,inflammatory factor levels.The he curative effect was assessed.Results The overall effective rate in the treatment group was significantly better than that in the control group(97.62%[41/42]vs 83.33%[35/42],[P<0.05],respectively).Significantly increased ADL and KSS scores,and significantly decreased VAS were detected in the two groups after treatment(P<0.05),and there were statistically significant differences in ADL,KSS and VAS between groups(P<0.05).After treatment,traditional Chinese medicine(TCM)symptom scores in groups were significantly decreased(P<0.05),which decreased notably in the treatment group compared with the control group(P<0.05).IL-6,TNF-α,IL-1 in groups were significantly decreased(P<0.05),which were more pronounced in the treatment group than in the control group(P<0.05).Conclusion For KOA patients,Yishen Zhuanggu Decoction combined with three-step massage and acupoint has obvious effect,which can significantly improve ADL,knee joint function,relieve pain,reduce the level of organism inflammatory factors.It is worthy of clinical promotion.
