中性粒细胞与淋巴细胞比值对糖尿病性玻璃体积血的预测价值

Predictive value of the neutrophil-to-lymphocyte ratio for risk of diabetic vitreous hemorrhage

目的:分析全血细胞计数衍生炎症标志物对糖尿病性玻璃体积血(DVH)的预测价值。方法:本回顾性病例对照研究纳入2020年1月至2021年2月在武汉大学人民医院眼科中心住院并接受手术治疗的41例DVH患者和41例年龄、性别1∶1匹配且无糖尿病性视网膜病变的糖尿病患者(对照组)。收集所有参与者的临床资料和实验室检测结果,记录并计算全血炎症指标。结果:DVH组的中性粒细胞与淋巴细胞比值(NLR)、中性粒细胞和血红蛋白与淋巴细胞比值(NHL)明显高于对照组(P<0.05)。血小板与淋巴细胞比值(PLR)和系统性免疫炎症指数(SII)在两组间无明显差异(P>0.05)。Logistic回归分析显示,NLR和NHL是糖尿病患者发生DVH的危险因素(P<0.05)。ROC曲线分析显示,NLR和NHL预测DVH发生的曲线下面积(AUC)分别为0.70和0.63。结论:NLR和NHL是预测DVH的简单而廉价的炎症标志物。NLR优于其他全血炎症标志物。

Objective:To evaluate the predictive value of systemic inflammatory markers derived from com-plete blood counts in patients with diabetic vitreous hemorrhage(DVH).Methods:This retrospective case-control study included 41 DVH patients and 41 age-and sex-matched diabetic patients without diabetic retinopathy(control group).Venous blood samples were collected from all participants for counts,and differences in inflammatory markers were recorded and compared.Results:In this study,neutrophil-to-lymphocyte ratio(NLR)and the ratio of neutrophils and hemoglobin to lymphocytes(NHL)values were significantly higher in the DVH group as compared with the control group(P<0.05),whereas there were no statistically significant differences in the platelet-to-lymphocyte ratio(PLR)and systemic immune-inflammation index(SII)(P>0.05).Logistic regression analysis showed that NLR(OR=29.756,95%CI:3.504-252.670,P=0.002)was a risk factor for DVH and NHL(OR=0.981,95%CI:0.967-0.995,P=0.007)was a protective factor.The receiver operating characteristic(ROC)curve was used to predict DVH.The area under the curve(AUC)was 0.70 for NLR and 0.63 for NHL.Conclusion:NLR and NHL were simple and inexpensive inflammatory biomarkers for predicting DVH.NLR was superior to other inflammatory biomarkers.
