超声内镜辅助下ESD治疗上消化道隆起性病变的临床研究

Clinical Study of Endoscopic Ultrasound Assisted ESD in the Treatment of Upper Gastrointestinal Tract Eminence

目的探究超声内镜辅助下内镜黏膜下剥离术(endoscopic submucosal dissectio,ESD)治疗上消化道隆起性病变的临床研究。方法随机选取2020年1月—2023年7月宿迁市沭阳医院收治的70例上消化道隆起性病变患者为研究对象,依照随机数表法分为对照组和观察组,每组35例。在超声内镜辅助下对照组采取黏膜切除术治疗法,观察组采取ESD治疗法,对比两组患者围术期各项指标、治疗效果以及并发症发病率。结果观察组手术时间为(34.21±6.53)min、术后下床时间为(2.14±0.36)d、恢复进食时间为(2.33±0.69)d、住院时间为(7.21±1.56)d均短于对照组,术中出血量为(90.35±19.57)mL少于对照组,差异有统计学意义(t=14.182、9.881、9.306、6.864、6.290,P<0.05);观察组的治疗效果高于对照组,差异有统计学意义(P<0.05),观察组的并发症发病率低于对照组,差异有统计学意义(P<0.05)。结论对于接诊的上消化道隆起性病变患者以超声内镜辅助下实施ESD治疗,可帮助患者提高治疗效果,缩短术后恢复时间,降低并发症发病率,对临床治疗具有重要意义。

Objective To explore the clinical study of endoscopic submucosal dissectio(ESD)assisted by endoscopic ultrasound in the treatment of upper gastrointestinal tract eminence.Methods A total of 70 patients with upper gastrointestinal protuberant lesions admitted to Shuyang Hospital of Suqian City from January 2020 to July 2023 were randomly selected as the research objects.According to the random number table method,they were divided into control group and observation group,with 35 cases in each group.The control group was treated with mucosal resection under the assistance of endoscopic ultrasonography,and the observation group was treated with ESD.The perioperative indexes,therapeutic effect and incidence of complications were compared between the two groups.Results The operation time was(34.21±6.53)min,postoperative ambulation time was(2.14±0.36)d,recovery eating time was(2.33±0.69)d and hospitalization time was(7.21±1.56)d in the observation group,which were shorter than those in the control group,and the intraoperative blood loss was(90.35±19.57)mL,which was less than that in the control group,the differences were statistically significant(t=14.182,9.881,9.306,6.864,6.290,P<0.05);the therapeutic effect of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion ESD therapy with the assistance of endoscopic ultrasound can help patients improve the therapeutic effect,shorten the postoperative recovery time and reduce the incidence of complications,which is of great significance for clinical treatment.
