雷火灸对中风后偏瘫患者患侧肢体功能、肌力和平衡能力的影响

Effects of Thunder Fire Moxibustion on the Function,Muscle Strength and Balance of the Affected Side of the Limb in Patients with Hemiplegia after Stroke

目的探讨对中风后偏瘫患者采用雷火灸对改善患侧肢体功能、肌力以及平衡能力的作用。方法方便选取2021年1月—2022年12月如皋市中医院收治的96例中风后偏瘫患者为研究对象,以随机数表法分为两组。对照组(n=48)予常规护理,观察组(n=48)则采用雷火灸干预。比较干预后傅格-梅尔运动评分(Fugl-Meyer Assessment,FMA)、改良巴塞尔指数(Modified Barthel Index,MBI)评估患侧肢体功能,以改良阿什沃思量表(Modified Ashworth Scale,MAS)、英国医学研究理事会肌力分级(MRC-Score,MRC)评估肌张力与肌力,另以伯格平衡量表(Berg Balance Scale,BBS)与站立行走测试(Time Up and Go-Test,TUGT)评估平衡能力。结果干预后观察组Fugl-Meyer上肢、下肢运动功能(45.12±6.10)分、(29.50±3.71)分,改良MBI指数(65.61±7.27)分,均高于对照组,差异有统计学意义(t=4.996、8.522、6.119,P<0.05)。干预后,观察组MAS量表评分低于对照组,MRC肌力分级高于对照组差异有统计学意义(P<0.05)。干预后在BBS评分、站立行走测试两项结果上,观察组评估结果均更优,差异有统计学意义(P<0.05)。结论雷火灸能提高中风后偏瘫患者肢体功能、肌力和平衡能力。

Objective To investigate the effects of thunder fire moxibustion on improving the function,muscle strength and balance of the affected side of the limbs in patients with hemiplegia after stroke.Methods A total of 96 patients with hemiplegia after stroke treated in Rugao Hospital of Traditional Chinese Medicine from January 2021 to December 2022 were conveniently selected as the research objects and they were divided into two groups according to the random number table method.The control group(n=48)was given routine nursing,and the observation group(n=48)was given lightning moxibustion intervention.After the intervention,Fugl-Meyer Assessment(FMA)and Modified Barthel Index(MBI)were compared to evaluate the function of the affected limb.Muscle tone and muscle strength were assessed using the Modified Ashworth Scale(MAS)and the Medical Research Council Muscle Strength Score(MRC-Score).The Berg Balance Scale(BBS)and the Time Up and Go-Test(TUGT)were used to assess balance ability.Results After intervention,Fugl-Meyer motor function of upper and lower limbs(45.12±6.10)points and(29.50±3.71)points and modified MBI index(65.61±7.27)points in observation group were higher than those in control group,and the differences were statistically significant(t=4.996,8.522,6.119,P<0.05).After intervention,MAS scale score of observation group was lower than control group,MRC muscle strength grade was higher than control group,and the differences were statistically significant(P<0.05).After intervention,the evaluation results of BBS score and standing and walking test were better in the observation group,and the differences were statistically significant(P<0.05).Conclusion Thunder fire moxibustion can improve limb function,muscle strength and balance in patients with hemiplegia after stroke.
