基于价值流程图的急性肠系膜上动脉栓塞患者急诊术前院内流程现状分析

Analysis of current status of preoperative in-hospital process for patients with acute superior mesenteric artery embolism based on value stream mapping

目的 调查急性肠系膜上动脉栓塞患者的救治流程患者急诊术前院内流程,并用价值流程图进行展示分析。方法 选取2022年1月—2023年1月某医院急诊入院治疗的10例急性肠系膜上动脉栓塞患者为研究对象。流程起始点为到达分诊台,结束点为进入手术室或介入中心。调查小组记录各个环节所用时间,绘制价值流程图。结果 急诊院内总流程时间为90~1 569 min,等待时间为30~526 min,工作时间为60~1 043 min,医师评估、家属参与决策、术前准备等待时间较长。结论 急性肠系膜上动脉栓塞患者的救治流程需要有大幅改善,亟待推广建设肠卒中绿色通道。

Objective To study the process of in-hospital patients with acute superior mesenteric artery embolism based on value stream mapping(VSM).Methods Ten patients with acute superior mesenteric artery embolism who admitted Xuanwu Hospital Capital Medical University from January 2022 to January 2023 were conducted on this study.A flowchart was designed from the nurse reception of emergency room to the operation room or intervention center.VSM was drawn and evaluated for each patient.Results The total process time to the emergency operation was 90-1569 minutes.The waiting time was 30-526 minutes,and the working time before operation was 60-1043 minutes.The waiting time for physician evaluation,family participation in decision-making,and preoperative preparation was relatively long.Conclusion The treatment process of patients with acute superior mesenteric artery embolism needs to be greatly improved,and a green channel for intestinal stroke needs to be promoted urgently.
