Experience of XIA Xinhua Using Modified Guizhi and Houpo Xingzi Decoction in the Treatment of Chronic Cough

介绍夏鑫华教授运用桂枝加厚朴杏子汤加减治疗慢性咳嗽的临床经验。夏鑫华教授深知岭南人体质,诊治慢性咳嗽注重体质辨证,重视肺脾阳气,以通阳散寒之桂枝加厚朴杏子汤主调肺脾;慢性咳嗽多为外感风寒邪气日久不愈迁延所致,多有风寒留恋、表邪未解之征,宜以桂枝加厚朴杏子汤辛温解表、散寒止嗽;临证每见寒热表现不明显者,主张“无热便是寒”的辨证理念,选用桂枝加厚朴杏子汤温肺散寒、降气止咳;慢性咳嗽多见肺胃同病,宜肺胃同治,桂枝加厚朴杏子汤亦属肺胃同治之代表方。

This paper introduces the clinical experience of Professor XIA Xinhua using modified Guizhi and Houpo Xingzi Decoction to treat chronic cough.Professor XIA is well aware of the physical constitution of people in the Lingnan area.In the diagnosis and treatment of chronic cough,he pays attention to the differentiation of constitutions and the yang qi in the lung and spleen and uses Guizhi and Houpo Xingzi Decoction which can unblock yang and dissipate cold to regulate the lung and spleen.Chronic cough is often caused by the external contraction of wind-cold pathogen which has failed to be cured in a long time,with the lingering wind-cold and unresolved exterior pathogen in most cases.It is advisable to use Guizhi and Houpo Xingzi Decoction to release the exterior with acrid-warm and dissipate cold to relieve cough.In the clinical treatment for those who do not show obvious signs of cold and heat,the dialectical concept of "no heat is cold" is advocated and Guizhi and Houpo Xingzi Decoction can be used to warm the lung and dissipate cold and direct qi downward and relieve cough.Chronic cough is more common in diseases of both the lung and stomach;it is recommended to treat the lung and stomach simultaneously,and Guizhi and Houpo Xingzi Decoction is such a representative formula.
