腹腔镜手术治疗女性腹股沟疝时子宫圆韧带处理方式研究进展

Research on the management of round uterine ligament during laparoscopic surgery for inguinal hernia in women

传统开放手术治疗女性腹股沟疝时大多数能避开子宫圆韧带完成手术操作,与开放式前入路手术不同,腹腔镜手术治疗腹股沟疝为后入路手术,其能清晰看见肌耻骨孔这一区域并对其进行修补.在女性腹股沟疝患者中,子宫圆韧带在肌耻骨孔下方深面走行,术中需对该区域完成修补可能会离断子宫圆韧带,术后关于子宫脱垂、不孕等并发症目前仍无相关研究提示与离断韧带相关,因而绝大多数人主张保留子宫圆韧带.本文就腹腔镜手术治疗女性腹股沟疝时子宫圆韧带的处理进行综述.

Unlike open anterior approach surgery,laparoscopic surgery for inguinal hernia is a posterior approach surgery,which allows for clear visualization of the area of the musculopubic foramen and repair of the area.In female inguinal hernia patients,the round uterine ligament runs deep below the musculopubic foramen,and intraoperative repair of this area may dissect the round uterine ligament.Postoperative complications,such as uterine prolapse and infertility,have not been shown to be associated with dissected ligaments,and therefore,preservation of the round uterine ligament is advocated by the majority of the population.This article provides a review of the management of the round uterine ligament during laparoscopic surgery for inguinal hernia in women.
