中药熏洗配合平衡椅子操对老年2型糖尿病周围神经病变患者下肢肌力及平衡功能的影响

Effects of traditional Chinese herbal fumigation combined with balance chair exercise on lower limb muscle strength and balance function of elderly patients with type 2 diabetes mellitus complicated with peripheral neuropathy

目的观察中药熏洗配合平衡椅子操对老年糖尿病周围神经病变(DPN)患者下肢肌力及平衡功能的影响。方法2020年7月—2023年7月于江苏省中医院收治的120例老年2型糖尿病(T2DM)合并DPN患者,随机分为研究组(n=60)和对照组(n=60)。对照组进行平衡椅子操训练,研究组在对照组的基础上联合中药熏洗。治疗8周后,比较2组的疗效、下肢肌力、平衡功能以及不良反应。结果治疗8周后,研究组总有效率高于对照组(75.00%vs 90.00%,P=0.031)。与对照组相比,研究组腓肠肌、胫骨前肌和股四头肌治疗前后的肌力差值较大;研究组起立-行走计时测试和单腿站立测试治疗前后的差值较大,差异有统计学意义(P<0.05)。2组不良反应发生率差异无统计学意义(6.67%vs 5.00%,P=0.697)。结论中药熏洗配合平衡椅子操可能改善老年T2DM合并DPN患者的下肢肌力与平衡功能。

Objective To observe the effects of traditional Chinese medicine fumigation combined with balanced chair exercise on lower limb muscle strength and balance function of elderly patients with diabetic peripheral neuropathy(DPN).Methods 120 patients with type 2 diabetes mellitus(T2DM)and DPN admitted to Traditional Chinese Medicine Hospital of Jiangsu Province from July 2020 to July 2023 were randomly divided into study group(n=60)and control group(n=60).The control group received balanced chair exercise training,and the study group received traditional Chinese medicine fumigation on the basis of the control group.After 8 weeks of treatment,the efficacy,lower limb muscle strength,balance function and adverse reactions were compared between the two groups.Results After 8 weeks of treatment,the total effective rate in the observation group was higher than that in the control group(75.00%vs 90.00%,P=0.031).Compared with control group,the differences of muscle strengths of gastrocnemius,tibialis anterior muscle and quadriceps femoris before and after treatment were greater in the study group;the difference between standing up and walking timing test and one-leg standing test before and after treatment were greater in the study group(P<0.05).There was no statistical significance in the incidence of adverse reactions between the two groups(6.67%vs 5.00%,P=0.697).Conclusion Traditional Chinese medicine fumigation combined with balanced chair exercise may improve the lower limb muscle strength and balance function of elderly patients with T2DM and DPN.
