单孔与多孔腹腔镜子宫肌瘤剥除术对子宫肌瘤患者的疗效比较

Comparison of the Efficacy of Single-hole and Multi-hole Laparoscopic Hysteromyoma Removal in Patients with Hysteromyoma

目的 探讨单孔与多孔腹腔镜子宫肌瘤剥除术治疗子宫肌瘤患者后的效果。方法 随机选取2020年3月—2022年12月邳州市人民医院收治的70例子宫肌瘤患者为研究对象,按照随机数表法分为对照组和观察组,各35例。对照组实施多孔腹腔镜子宫肌瘤剥除术治疗,观察组实施单孔腹腔镜子宫肌瘤剥除术治疗。对比两组临床相关指标、疼痛状态(Visual Analogue Scale, VAS)、切口美观度(cutaway small, CS)、并发症。结果 两组住院时间比较,差异无统计学意义(P>0.05)。观察组手术时间、肛门排气时间、术中出血量、术后引流量均优于对照组,差异有统计学意义(P<0.05)。观察组VAS评分低于对照组,CS评分高于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率(2.86%)低于对照组(17.14%),差异有统计学意义(χ^(2)=3.968,P<0.05)。结论 通过单孔腹腔镜子宫肌瘤剥除术治疗子宫肌瘤患者,可以缩短肛门排气时间,减少术中出血量以及术后引流量,缓解疼痛,提升切口美观度,降低并发症发生率。

Objective To investigate the effect of single-hole and multi-hole laparoscopic hysteromyoma stripping in the treatment of patients with hysteromyoma.Methods 70 patients with uterine fibroids admitted to the Pizhou People's Hospital from March 2020 to December 2022 were selected as the research subjects.They were randomly divided into a control group and an observation group,with 35 cases in each group,using the random number table method.The control group underwent porous laparoscopic myomectomy,while the observation group underwent single port laparoscopic myomectomy.Compare two groups of clinically relevant indicators,Visual Analogue Scale(VAS),incision aesthetics(CS),and complications.Results There was no statistically significant difference in hospital stay between the two groups(P>0.05).The operative time,anal exhaust time,intraoperative blood loss and postoperative drainage volume in the observation group were better than those in the control group,and the differences were statistically significant(P<0.05).VAS score of observation group was lower than control group,CS score was higher than control group,the difference was statistically significant(P<0.05).The complication rate of observation group(2.86%)was lower than that of control group(17.14%),and the difference was statistically significant(χ^(2)=3.968,P<0.05).Conclusion The treatment of patients with uterine fibroids by single-hole laparoscopic fibroid removal can shorten the anal exhaust time,reduce the amount of intraoperative blood loss and postoperative drainage flow,relieve pain,improve incision aesthetics,and reduce the incidence of complications.
