关节镜下清理术联合术后全程康复治疗膝骨关节炎疾病的临床价值分析

Clinical Value of Arthroscopic Debridement Combined with Postoperative Rehabili tation in the Treatment of Knee Osteoarthritis

目的 分析膝骨关节炎患者接受关节镜下清理术+术后全程康复治疗的价值。方法 随机选取2020年1月—2023年4月山东健康集团枣庄医院关节与运动医学科收治的50例膝骨关节炎疾病患者为研究对象,通过双盲法分为常规术后组(25例,关节镜下清理术+常规康复治疗方法)与全程康复组(25例,关节镜下清理术+全程康复治疗方法),比较两组日本骨科协会评估治疗分数(Japanese Orthopedics Association, JOA)评分、下肢功能状况、视觉模拟评分法(Visual Analogue Scale, VAS)、独立生活能力、不良事件情况。结果 治疗后全程康复组的关节活动度(27.41±0.19)分,疼痛和步行距离(22.30±1.03)分,上下楼用扶手(15.36±0.41)分均高于常规术后组,差异有统计学意义(t=44.067、7.760、20.669,P<0.05)。全程康复组不良事件情况发生率为4.00%低于常规术后组的16.00%,差异有统计学意义(χ^(2)=4.347,P=0.037)。结论 膝骨关节炎患者接受关节镜下清理术+术后全程康复治疗措施更加有利于关节功能恢复,改善下肢功能,缓解术后关节疼痛状况,降低不良预后结局发生率。

Objective To analyze the value of arthroscopic debridement plus postoperative rehabilitation in patients with knee osteoarthritis.Methods Fifty patients with knee osteoarthritis admitted to the Joint and Sports Medicine Department of Zaozhuang Hospital of Shandong Health Group from January 2020 to April 2023 were randomly selected as the research subjects.They were divided into a conventional postoperative group(25 cases,arthroscopic cleaning surgery+conventional rehabilitation treatment)and a full rehabilitation group(25 cases,arthroscopic cleaning surgery+full rehabilitation treatment)using a double-blind method,Compared the Japanese Orthopedics Association(JOA)score,lower limb function,Visual Analog Scale(VAS),independent living ability,and adverse events between two groups.Results The joint range of motion(27.41±0.19)points,pain and walking distance(22.30±1.03)points,and handrail up and down(15.36±0.41)points scores of the full rehabilitation group after treatment were higher than those of the conventional postoperative group,and the differences were statistically significant(t=44.067,7.760,20.669,all P<0.05).The incidence of adverse events in the full rehabilitation group was 4.00%,which was lower than the 16.00%in the conventional postoperative group,and the difference was statistically significant(χ^(2)=4.347,P=0.037).Conclusion Arthroscopic debridement plus postoperative rehabilitation for patients with knee osteoarthritis is more conducive to the recovery of joint function,improving lower limb function,alleviating postoperative joint pain,and reducing the incidence of adverse prognosis.
