脑电生物反馈治疗对脑卒中后注意障碍的疗效分析

摘要 目的 研究脑电生物反馈治疗对脑卒中后注意障碍的疗效。方法 方便选取2021年8月—2023年7月郴州市第一人民医院收治的85例脑卒中后注意障碍患者为研究对象,按照随机数字表法分为对照组和观察组,对照组(n=43)接受常规康复治疗,研究组(n=42)在对照组基础上接受脑电生物反馈治疗,比较两组患者治疗效果。结果 治疗4周后,研究组θ波(20.84±1.89)Hz下降幅度大于对照组的(25.85±2.16)Hz,β波(6.98±0.97)Hz、SMR波(8.32±0.98)Hz升高幅度大于对照组的(6.11±0.41)、(7.21±0.99)Hz,差异有统计学意义(t=11.370、5.408、5.194,P均<0.05);治疗4周后,研究组关联性负变(contingent negative variation, CNV)期待波平均波幅(14.76±2.96)μv升高幅度大于对照组(10.86±4.95)μv,潜伏期(358.78±23.87)ms减少幅度小于对照组的(469.85±70.15)ms,面积(18 585.49±1 226.89)Cz增加幅度大于对照组的(13 895.84±3 651.55),差异有统计学意义(t=4.395、9.725、7.898,P均<0.05);治疗4周后,研究组数字广度测验(digital span test, DST)正向(6.26±1.04)分、逆向(3.98±0.89)分评分高于对照组的(4.46±1.21)分、(3.25±0.75)分,数字警觉测验(digit vigilance test, DVT)正确划掉数(109.59±17.84)个大于对照组的(96.59±18.96)个,速度(289.84±38.67)s优于对照组的(313.48±49.78)s,差异有统计学意义(t=7.348、4.093、3.254、2.438,P均<0.05)。结论 脑电生物反馈治疗脑卒中后注意障碍效果良好。 Objective To study the effect of EEG biofeedback therapy on attention disorder after stroke.Methods A total of 85 patients with attention deficit after stroke treated in the Chenzhou First People's Hospital from August 2021 to July 2023 were conveniently selected as the research objects.According to the random number table method,they were divided into control group and study group.The control group(n=43)received routine rehabilitation treatment,and the study group(n=42)received electroencephalogram biofeedback therapy on the basis of the control group.Results After 4 weeks of treatment,θwave(20.84±1.89)Hz decreased more than that of control group(25.85±2.16)Hz,βwave(6.98±0.97)Hz,SMR wave(8.32±0.98)Hz increased more than that of control group(6.11±0.41)Hz and(7.21±0.99)Hz,the differences were statistically significant(t=11.370,5.408,5.194,all P<0.05).After 4 weeks of treatment,the average amplitude of contingent negative variation(CNV)was higher in the study group(14.76±2.96)μv than in the control group(10.86±4.95)μv,the latency period(358.78±23.87)ms decreased less than that of the control group(469.85±70.15)ms,the area(18585.49±1226.89)Cz increased more than that of the control group(13895.84±3651.55)Cz,and the differences were statistically significant(t=4.395,9.725,7.898,all P<0.05).After 4 weeks of treatment,the positive(6.26±1.04)points and inverse(3.98±0.89)points scores of digital span test(DST)in the study group were higher than those in the control group(4.46±1.21)points and(3.25±0.75)points.digit vigilance test(DVT)the number of correct scratches(109.59±17.84)s was higher than that of the control group(96.59±18.96).The speed(289.84±38.67)s was better than that of control group(313.48±49.78)s,and the differences were statistically significant(t=7.348,4.093,3.254,2.438,all P<0.05).Conclusion The effect of EEG biofeedback on attention disorder after stroke is good,and it is worth promoting.
