体外反搏联合心脏康复治疗对经皮冠状动脉介入术后患者心肺功能的影响

Effect of external counterpulsation combined with cardiac rehabilitation on cardiopulmonary function in patients after percutaneous coronary intervention

目的评估体外反搏联合心脏康复治疗对经皮冠状动脉介入术后患者心肺功能的影响。方法选择2021年1月至2022年12月在上海市杨浦区控江医院心血管内科就诊的已行冠状动脉造影并行冠状动脉支架植入术后的患者为研究对象,共计150例。将患者随机数字表法分为对照组,心脏康复组及体外反搏联合心脏康复组,每组50例。分别于患者入组时、入组6个月后进行心肺运动试验(CPET)测试及生活质量(SF-36)量表测试,比较各组治疗的情况。结果和治疗前相比,体外反搏联合心脏康复组和心脏康复组的最大摄氧量(VO2max)、最大公斤摄氧量(VO2max/kg)、氧脉(VO2/HR)、无氧代谢阈值(AT)、一秒率(FEV1/FVC)及深吸气量(IC)显著增加。治疗后体外反搏联合心脏康复组的VO2max、VO2max/kg、AT、代谢当量及IC均高于心脏康复组和对照组(P<0.05)。和治疗前相比,体外反搏联合心脏康复组在治疗后,除躯体疼痛外其他指标均得到显著改善(P<0.05),且在一般健康状况、精力、精神健康、健康变化方面优于对照组和心脏康复组(P<0.05)。结论体外反搏联合心脏康复治疗对PCI术后患者的心肺功能及生活质量的提高效果显著。

Objective To evaluate the effect of external counterpulsation(ECP)combined with cardiac rehabilitation(CR)treatment on cardiopulmonary function of patients after percutaneous coronary intervention(PCI).Methods A total of 150 patients who had undergone coronary angiography and coronary stenting in the Department of Cardiology,Shanghai Kongjiang Hospital in Yangpu District,from January 2021 to December 2022 were studied.Patients were randomly divided into the control group,the cardiac rehabilitation group,and the ECP combined cardiac rehabilitation group.Fifty people in each group were measured by CPET and SF-36 scale 6 months after the patients were enrolled,respectively,then compare the treatment status of each group.Results Compared with before treatment,the VO2max,VO2max/kg,VO2/HR,anaerobic metabolism threshold(AT),metabolic equivalent(METs),FEV1/FVC,and deep inspiratory capacity(IC)in the ECP combined cardiac rehabilitation group and cardiac rehabilitation group were significantly increased(P<0.05).After treatment,VO2max,VO2max/kg,AT,METs,and IC in the ECP combined cardiac rehabilitation group were significantly higher than those in cardiac rehabilitation group and control group(P<0.05).Compared with before treatment,the ECP combined cardiac rehabilitation group showed significant improvement in all indicators except physical pain(P<0.05),and in general health status,energy,mental health and health changes were significantly better than those of the control group and the CR group(P<0.05).Conclusion ECP combined with cardiac rehabilitation has a significant effect on the improvement of cardiopulmonary function and quality of life of patients after PCI.
