高压氧联合熏洗方治疗脊柱骨折46例疗效分析

Analysis of therapeutic effects of hyperbaric oxygen therapy combined with fumigation and bath formula in 46 cases of spinal fracture

目的探讨高压氧(HBO)联合熏洗方治疗脊柱骨折患者的临床疗效。方法选取2022年1月至2023年1月宁德师范学院附属宁德市医院脊柱外科收治的92例脊柱骨折患者作为研究对象。按照随机数字表法分为观察组和对照组, 每组46例。2组患者术后均予以HBO治疗, 观察组在对照组的基础上联合中药熏洗。比较2组患者临床优良率、神经功能、脊柱功能、血清疼痛因子及血液流变学指标的变化。结果观察组患者治疗后临床优良率(93.48%)明显高于对照组(80.43%), 差异有统计学意义(P<0.05)。与对照组治疗后比较, 观察组患者治疗后全血高切黏度、全血低切黏度、血浆黏度显著降低(P<0.01);观察组患者治疗后Frankel评分及美国国立卫生研究院卒中量表(NIHSS)评分明显低于对照组(P<0.01);观察组治疗后血清5-羟色胺(5-HT)和P物质水平明显低于对照组(P<0.01);观察组治疗后Oswestry功能障碍指数(ODI)评分明显降低, Prolo腰椎功能量表评分(Prolo评分)明显升高(P<0.001)。结论 HBO联合熏洗方治疗可明显提高脊柱骨折患者的临床疗效, 明显改善患者的神经功能和脊柱功能。

Objective To evaluate the clinical efficacy of hyperbaric oxygen(HBO)combined with fumigation and bath formula in patients with spinal fractures.Methods A total of 92 patients with spinal fracture admitted to the Department of Spinal Surgery of Ningde Municipal Hospital of Ningde Normal University from January 2022 to January 2023 were selected as the study subjects.They were divided into observation group and control group according to the random number table method,with 46 cases in each group.Both groups received HBO therapy after surgery,while the observation group additionally received Traditional Chinese Medicine fumigation and bath formula based on the treatment received by the control group.The rate of excellent and good clinical outcome,neurological function,spinal function,serum pain factors,and changes in hemorheological indicators were compared between the two groups.Results The rate of excellent and good clinical outcome(93.48%)of the observation group after treatment was significantly higher than that of the control group(80.43%),and the difference was statistically significant(P<0.05).Compared with the control group after treatment,the observation group showed significant decreases of whole blood high shear viscosity,whole blood low shear viscosity,and plasma viscosity(P<0.01).The Frankel score and National Institute of Health stroke scale(NIHSS)score of the observation group were significantly lower than those of the control group after treatment(P<0.01).After treatment,the levels of serum 5-hydroxytryptamine(5-HT)and substance P in the observation group were significantly lower than those in the control group(P<0.01).Compared with the control group after treatment,the observation group showed a significant decrease in Oswestry Disability Index(ODI)score and a significant increase in Prolo Lumbar Spine Function Questionnaire score(Prolo score)(P<0.001).Conclusion HBO therapy combined with fumigation and bath formula can significantly improve the clinical efficacy of spinal fracture treatment,and obviously promote the patients’neurological and spinal functions.
