摘要
目的:探讨三维超声左心室造影(Three-dimensional contrast-enhanced,3D-CE)相关参数联合超声心动图(Ultrasound Cardiogram,UCG)评估慢性心力衰竭患者(Chronic heart failure,CHF)病情程度。方法:选取2021年5月至2022年10月我院收治的78例CHF患者作为研究对象,均接受3D-CE、UCG检测,比较左心室第16节段、第12节段达最小收缩末容积时间的标准差(Time to minimum systolic volume,Tmsv)即Tmsv16-SD、Tmsv12-SD,左室射血分数(Left ventricular ejection fraction,LVEF)、左室舒张末期内径(Left ventricular end diastolic diameter,LVEDD),并绘制受试者工作特征曲线(Receiver operating curve,ROC),分析该法评估CHF患者病情程度的价值。结果:所有CHF患者中,美国纽约心脏协会(The New York Heart Association,NYHA)分级Ⅱ级28例、Ⅲ级31例、Ⅳ级19例。Ⅱ级Tmsv16-SD、Tmsv12-SD、LVEDD值最低,LVEF值最高(P<0.05)。绘制ROC曲线结果显示,3D-CE、UCG单独及联合评估CHF病情程度的曲线下面积(Area under curve,AUC)均>0.7,联合评估价值最高。结论:3D-CE联合UCG在CHF患者病情程度评估中有较高应用价值。
Objective:To investigate the relevant parameters of Three-dimensional left ventriculography(3D-CE)combined with Ultrasound Cardiogram.(UCG)to assess the severity of Chronic heart failure(CHF)in patients.Methods:78 patients with CHF admitted to our hospital from May 2021 to October 2022 were selected as research objects,all of whom were tested with 3D-CE and UCG,and the standard deviation of Time to minimum systolic volume at left ventricular segment 16 and 12 was compared.Tmsv,Tmsv16-SD,Tmsv12-SD,Left ventricular ejection fraction(LVEF),Left ventricular end diastolic diameter(LVEDD),and Receiver operating curve(ROC)were drawn.To analyze the value of this method in assessing the severity of CHF.Results:Among all patients with CHF,28 cases were classified by The New York Heart Association(NYHA)as grade II,31 cases as grade III,and 19 cases as grade IV.Tmsv16-SD,Tmsv12-SD and LVEDD values of classⅡwere the lowest,and LVEF values were the highest(P<0.05).ROC curve results showed that the Area under curve(AUC)of 3D-CE and UCG alone and combined in assessing the severity of CHF was>0.7,and the combined assessment value was the highest.Conclusion:3D-CE combined with UCG has high application value in the evaluation of the disease degree of CHF patients.
