超声引导下关节腔内注射治疗颞下颌关节紊乱症的疗效分析 被引量:1

Analysis of the therapeutic effect of ultrasound-guided intraarticular injection in the treatment of temporomandibular joint disorders

摘要 目的 探究超声引导下关节腔内注射治疗颞下颌关节紊乱症的疗效.方法 选取2022年8月-2023年11月期间就诊于台山市中医院的67例颞下颌关节紊乱症患者为研究对象,按随机数字表法分为对照组(33例)及实验组(34例),对照组实施直接关节腔内注射曲安奈德治疗,实验组实施超声引导下关节腔内注射曲安奈德治疗.比较两组患者疗效、穿刺中及治疗后并发症情况、患者的开口度、疼痛指数以及其满意度.结果 实验组临床治疗总有效率为94.12%,高于对照组的75.76%(P<0.05);实验组穿刺中及治疗后并发症总发生率为2.94%,低于对照组的27.27%(P<0.05).两组治疗前患者的开口度对比,差异无统计学意义(P>0.05);两组治疗后1个月、3个月患者的开口度大于治疗前(P<0.05),且实验组大于对照组(P<0.05).两组治疗前视觉模拟疼痛评分(VAS)对比,差异无统计学意义(P>0.05);两组治疗后1个月、3个月VAS评分低于治疗前(P<0.05),且实验组的评分低于对照组(P<0.05);实验组满意度评分高于对照组(P<0.05).结论 超声引导下关节腔内注射治疗颞下颌关节紊乱症可以改善患者开口度,减轻其疼痛程度,提高治疗效果和满意度,且可以减少穿刺中及治疗后并发症发生率,提高治疗安全性. Objective To explore the therapeutic effect of ultrasound-guided intraarticular injection in the treatment of temporomandibular joint disorders.Methods From August 2022 to November 2023,67 patients with temporomandibular joint disorder in our hospital were collected as the subjects of this study,and assigned to the control group(33 cases)and the experimental group(34 cases)according to random number table method.The control group received direct intra-articular injection of triamcinolone alone.The experimental group was treated with intraarticular triamcinolone injection under ultrasound guidance.The efficacy,complications during puncture and after treatment,patients'opening degree,pain index and satisfaction were compared between the two groups.Results The total effective rate of the experimental group was 94.12%,higher than that of the control group(75.76%)(P<0.05).The total complication rate of the experimental group was 2.94%,which was lower than that of the control group(27.27%)(P<0.05).There was no significant dfference in the opening degree between the two groups before treatment(P>0.05).The opening degree of the two groups 1 month and 3 months after treatment was greater than that before treatment(P<0.05),and the experimental group was greater than the control group(P<0.05).There was no significant difference in visual analogue pain scale(VAS)between the two groups before treatment(P>0.05).VAS scores 1 month and 3 months after treatment were lower than before treatment(P<0.05),and scores in the experimental group were lower than those in the control group(P<0.05).The satisfaction score of the experimental group was higher than that of the control group(P<0.05).Conclusion Ultrasound-guided intraarticular injection for temporomandibular joint disorders can improve patients'opening degree,reduce their pain,improve treatment effect and satisfaction,reduce complications during puncture and after treatment,and improve treatment safety.
