类风湿关节炎患者外周血单个核细胞TNIP1的表达水平与主要临床指标的相关性分析

摘要 目的探讨类风湿关节炎(rheumatoid arthritis,RA)患者外周血中CD14^(+)单核细胞、CD4^(+)T细胞、CD8^(+)T细胞、调节性T细胞(regulator T cell,Treg)中肿瘤坏死因子诱导蛋白3相互作用蛋白1(tumor necrosis factor alpha-induced protein 3-interacting protein 1,TNIP1)的表达与类风湿因子(rheumatoid factor,RF)、抗环瓜氨酸肽(cyclic citrulline peptide,CCP)抗体等主要临床指标的相关性。方法选取桂林医学院附属医院62例RA患者和39例健康体检者(health donor,HD),RA患者住院期间(HD于同期)抽取外周静脉血。采用流式细胞术检测HD和RA患者外周血中CD14^(+)单核细胞、CD4^(+)T细胞、CD8^(+)T细胞、CD4^(+)Foxp3^(+)Treg细胞TNIP1表达水平并转换为Z-score,并对比HD和RA患者TNIP1 Z-score;分析RA患者TNIP1 Z-score与RF、抗CCP抗体等主要临床指标的相关性。结果RA患者外周血中CD14^(+)单核细胞TNIP1表达水平与健康体检者差异无统计学意义,CD4^(+)T细胞、CD8^(+)T细胞、CD4^(+)Foxp3^(+)Treg细胞TNIP1表达水平显著高于健康体检者(P<0.05);RA患者CD4^(+)T细胞、CD8^(+)T细胞、CD4^(+)Foxp3^(+)Treg细胞TNIP1表达水平与RF呈正相关(P<0.01、P<0.01、P<0.05);RA患者CD4^(+)T细胞、CD8^(+)T细胞TNIP1 Z-score与RF呈正向线性相关(P<0.01)。结论RA患者TNIP1的高表达可能增强CD4^(+)T细胞、CD8^(+)T细胞促炎的能力,而抑制CD4^(+)Foxp3^(+)Treg细胞抑炎的能力,RA患者TNIP1高表达可能促进病程的发展。 Objective To determine the correlation of expressions of tumor necrosis factor alpha-induced protein 3-interacting protein 1(TNIP1)on CD14^(+)monocytes,CD4^(+)T cells,CD8^(+)T cells,and regulatory T cells(Tregs)in the peripheral blood of patients with rheumatoid arthritis(RA)as a function of the main clinical parameters including rheumatoid factor(RF)and anti-cyclic citrulline peptide(CCP)antibodies.Methods A total of 62 patients with RA and 39 healthy donor(HD)from the Affiliated Hospital of Guilin Medical University were selected for the study,and peripheral venous blood was drawn from RA patients during their hospitalization(the HD were during the same period).TNIP1 Z-scores were compared between RA patients and HD individuals.Additionally,the correlation between RA patients’TNIP1 Z-scores and major clinical indicators including rheumatoid factor(RF)and anti-CCP antibodies was examined.Results TNIP1 expression levels in CD4^(+)T cells,CD8^(+)T cells,and CD4^(+)Foxp3^(+)regulatory T cells were significantly higher in RA patients than in healthy donors(P<0.05).Positive correlations were identified between TNIP1 expression levels in CD4^(+)T cells,CD8^(+)T cells,CD4^(+)Foxp3^(+)regulatory T cells and RF levels(P<0.01,P<0.01,P<0.05).Moreover,significant positive linear correlations were found between TNIP1 Z-scores in CD4^(+)T cells,CD8^(+)T cells and RF levels in RA patients(P<0.01).Conclusion TNIP1 expressions in CD4^(+)T cells,CD8^(+)T cells,and CD4^(+)Foxp3^(+)regulatory T cells in PBMCs of RA patients are higher than that in healthy donors.Furthermore,TNIP1 expression levels in CD4^(+)T cells,CD8^(+)T cells,and CD4^(+)Foxp3^(+)regulatory T cells are associated with RF levels in RA patients.
