CT及磁共振血管造影用于颅内动脉瘤诊断效能分析

摘要 目的分析CT及磁共振(MR)血管造影用于颅内动脉瘤诊断的效能。方法回顾性分析本院2019年1月至2021年12月收治的疑似颅内动脉瘤患者60例,并分为2组,即观察组(30例)和对照组(30例)。对照组行MRI,观察组行CT血管造影,并以数字减影血管造影(DSA)诊断结果为依据对比2组诊断灵敏度、特异度及准确度。结果据DSA诊断结果显示,60例疑似颅内动脉瘤患者中,阳性患者50例,阴性患者10例,其中对照组检出21例真阳,3例真阴,观察组检出23例真阳,4例真阴;对照组诊断灵敏度、特异度及准确度分别为87.0%、42.9%、80.0%,观察组诊断灵敏度、特异度及准确度分别为95.8%、66.7%、90.0%,2组比较差异无统计学意义(P>0.05);50例确诊患者中,小型动脉瘤诊断符合率为对照组43.8%,观察组50.0%;中型动脉瘤诊断符合率为对照组43.3%,观察组46.7%;大型动脉瘤诊断符合率为对照组100.0%,观察组100.0%;2组比较差异无统计学意义(P>0.05)。结论CT及MR血管造影均具有较高的诊断价值,但相对而言更推荐采用CT血管造影。 Objective To analyze the effectiveness of CT and magnetic resonance(MR)angiography in the diagnosis of intracranial aneurysms.Methods A retrospective analysis was conducted on 60 suspected intracranial aneurysm patients admitted to our hospital from January 2019 to December 2021,and they were divided into two groups:the observation group(n=30)and the control group(n=30).The control group underwent MR angiography,while the observation group underwent CT angiography.The diagnostic sensitivity,specificity,and accuracy of the two groups were compared based on the diagnostic results of digital subtraction angiography(DSA).Results According to the DSA diagnosis results,among the 60 suspected intracranial aneurysm patients,50 were positive and 10 were negative.Among them,the control group detected 21 cases of true positive and 3 cases of true negative,while the observation group detected 23 cases of true positive and 4 cases of true negative;The diagnostic sensitivity,specificity,and accuracy of the control group were 87.0%,42.9%,and 80.0%respectively.The diagnostic sensitivity,specificity,and accuracy of the observation group were 95.8%,66.7%,and 90.0%,respectively.There were no significant diffrencce between the two groups(P>0.05).Among the 50 confirmed patients,the diagnostic accuracy of small aneurysms was 43.8%in the control group and 50.0%in the observation group;The diagnostic accuracy of medium-sized aneurysms was 43.3%in the control group and 46.7%in the observation group;The diagnostic accuracy of large aneurysms was 100.0%in the control group and 100.0%in the observation group;There were no significant difference between the two groups(P>0.05).Conclusion Both CT and MR angiography have high diagnostic value,but relatively speaking,CT angiography is more recommended.
