认知行为疗法联合氟西汀治疗青少年抑郁症疗效研究

Effect of cognitive behavioral therapy combined with fluoxetine in the treatment of adolescent depression

摘要 目的观察认知行为疗法联合氟西汀治疗青少年抑郁症的临床效果。方法选取2022年1月至2023年1月南阳市第四人民医院收治的122例青少年抑郁症患者作为研究对象,采用随机数表法将患者分为对照组和观察组各61例,对照组患者给予氟西汀治疗,观察组患者给予认知行为疗法联合氟西汀治疗,两组均治疗3个月,比较两组患者的疗效、以及治疗前后抑郁症状、抗抑郁药副反应量表(SERS)评分、记忆情况以及生活质量。结果观察组患者的治疗总有效率为93.44%,明显高于对照组的80.33%,差异有统计学意义(P<0.05);治疗后两组患者汉密顿抑郁量表(HAMD)评分均降低,且观察组患者HAMD评分为(8.32±2.11)分,明显低于对照组的(15.48±5.66)分,差异有统计学意义(P<0.05);治疗1周后及治疗4周后观察组患者抗抑郁药副反应量表(SERS)评分分别为(10.61±3.12)分、(7.63±2.17)分,明显低于对照组的(13.63±3.56)分、(9.58±2.62)分,差异有统计学意义(P<0.05);治疗后两组图片回忆、联想学习、理解记忆、背数评分均升高,且观察组图片回忆、联想学习、理解记忆、背数评分分别为(7.28±2.02)分、(5.03±1.38)分、(6.77±1.63)分、(6.91±1.66)分,明显高于对照组的(6.15±1.77)分、(4.13±1.14)分、(6.01±1.38)分、(6.08±1.73)分,差异有统计学意义(P<0.05);治疗后两组患者心理功能、躯体功能、社会功能、物质生活评分均升高,且观察组心理功能、躯体功能、社会功能、物质生活评分分别为(78.65±8.33)分、(76.23±9.21)分、(69.88±6.94)分、(75.34±9.52)分,明显高于对照组的(70.22±8.34)分、(66.52±9.45)分、(63.21±5.42)分、(70.32±9.75)分,差异均有统计学意义(P<0.05)。结论认知行为疗法联合氟西汀治疗青少年抑郁症能显著改善抑郁症状,减轻患者副反应,同时可改善患者记忆能力并提高生活质量,临床应用效果显著。 Objective To observe the clinical effect of cognitive behavioral therapy combined with fluoxetine in the treatment of adolescent depression.Methods A total of 122 adolescents with depression admitted to Nanyang Fourth People's Hospital from January 2022 to January 2023 were selected as the research subjects.Using the random number table method,they were divided into the control group and the observation group,with 61 cases in each group.The control group was treated with fluoxetine,and the observation group was treated with cognitive behavioral therapy combined with fluoxetine.Both groups received 3 months of treatment.Comparison between groups was made from perspectives of efficacy,depressive symptoms,the Rating Scale for Side Effects(SERS)scores,memory,and quality of life before and after treatment.Results The total response rate of the observation group(93.44%)was significantly higher than that in the control group(80.33%),and the difference was statistically significant(P<0.05).After treatment,the Hamilton Depression Scale(HAMD)scores of both groups decreased;HAMD score of the observation group was(8.32±2.11)points,significantly lower than(15.48±5.66)points of the control group;the differences were statistically significant(P<0.05).After 1 week and 4 weeks of treatment,the SERS scores of the observation group(10.61±3.12)points and(7.63±2.17)points,significantly lower than(13.63±3.56)points and(9.58±2.62)points of the control group(P<0.05).After treatment,the scores for picture recall,associative learning,understanding memory,and memorizing numbers of both groups increased;the scores of the observation group were(7.28±2.02)points,(5.03±1.38)points,(6.77±1.63)points,and(6.91±1.66)points,significantly higher than(6.15±1.77)points,(4.13±1.14)points,(6.01±1.38)points,and(6.08±1.73)points of the control group;the differences were statistically significant(P<0.05).After treatment,the scores for psychological function,physical function,social function,and material life of both groups increased;the scores of the observation group were(78.65±8.33)points,(76.23±9.21)points,(69.88±6.94)points,and(75.34±9.52)points,significantly higher than(70.22±8.34)points,(66.52±9.45)points,(63.21±5.42)points,and(70.32±9.75)points of the control group;the differences were statistically significant(P<0.05).Conclusion Cognitive behavioral therapy combined with fluoxetine is effective in the treatment of adolescent depression,which can significantly improve the patients’depressive state,enhance the efficacy,alleviate side effects,and improve their memory ability and quality of life.
