白蛋白/碱性磷酸酶与宫颈癌化疗患者预后的相关性

Correlation Between Albumin/alkaline Phosphatase and Prognosis of Patients withCervical Cancer Underwent Chemotherapy

目的 探讨白蛋白(ALB)/碱性磷酸酶(ALP)与宫颈癌化疗患者预后的相关性。方法 回顾性收集120例宫颈癌患者临床资料,所有患者接受4个周期的顺铂联合紫杉醇化疗。根据化疗周期完成后第30天时疗效评定结果将患者分为预后良好组(n=101)与预后不良组(n=19)。对比2组患者年龄、病程、疾病类型、疾病分期等基线资料;对比化疗前1周内所测的ALB、ALP及其他实验室指标,并计算ALB/ALP值。通过点二列相关性分析和受试者工作曲线(ROC)分析ALB/ALP与宫颈癌化疗患者预后的关系。结果 预后不良组肿瘤直径、ALP水平高于预后良好组,Ⅲ期与ⅣA期患者占比显著高于预后良好组,ALB、ALB/ALP显著低于预后良好组,差异有统计学意义(P<0.05)。经点二列相关性分析,ALB、ALP、ALB/ALP与宫颈癌化疗患者不良预后发生有关(P<0.05)。绘制受试者工作曲线(ROC),结果显示,ALB、ALP、ALB/ALP预测宫颈癌化疗患者不良预后的曲线下面积(AUC)值分别为0.804、0.215、0.862,ALB/ALP预测价值最高。结论 ALB/ALP与宫颈癌化疗患者不良预后间呈负相关,其值越低,不良预后发生可能性越高。

Objective To investigate the relationship between albumin(ALB)/alkaline phosphatase(ALP)and the prognosis of patients with cervical cancer who underwent chemotherapy.Methods Retrospective analysis was conducted to collect clinical data of 120 cervical cancer patients who underwent chemotherapy treatment.All patients received 4 cycles of cisplatin combined with paclitaxel chemotherapy.According to the efficacy evaluation results on the 30th day after the completion of the chemotherapy cycle,patients were divided into a good prognosis group(n=101)and a poor prognosis group(n=19).The baseline data such as age,course of disease,type of disease and stage of disease were compared between the 2 groups.The ALB,ALP and other laboratory indexes measured within 1 week before chemotherapy were compared,and the ALB/ALP value was calculated.The relationship between ALB/ALP and the prognosis of patients with cervical cancer chemotherapy was analyzed by point two column correlation analysis and receiver operating curve(ROC).Results The tumor diameter,ALP,and the proportion of stages III and IVA in the poor prognosis group were significantly higher than those in the good prognosis group,while ALB and ALB/ALP were significantly lower than those in the good prognosis group,with a statistically significant difference(P<0.05).Through point two column correlation analysis,ALB,ALP,ALB/ALP were associated with poor prognosis in cervical cancer patients underwent chemotherapy(P<0.05).The receiver operating curve(ROC)was drawn.The results showed that the area under the curve(AUC)values of ALB,ALP and ALB/ALP in predicting the poor prognosis of patients with cervical cancer chemotherapy were 0.804,0.215 and 0.862,respectively,and ALB/ALP had the highest predictive value.Conclusion There is a negative correlation between ALB/ALP and poor prognosis in cervical cancer patients underwent chemotherapy.The lower the value,the higher the likelihood of poor prognosis.
