基于中医传承辅助平台探析中医药治疗帕金森病抑郁的用药规律

Basing on Auxiliary Platform of Traditional Chinese Medicine Inheritance to Explore Medication Pattern of Depression in Parkinson's Disease with Traditional Chinese Medicine

目的:基于中医传承辅助平台(V2.5.0),探析帕金森病抑郁的用药规律,为中医药治疗帕金森病抑郁提供参考。方法:搜索中国期刊全文数据库(CNKI)、中国学术期刊数据库(万方数据)、中文科技期刊全文数据库(VIP)建库至2022年12月运用中药复方治疗帕金森病抑郁的文献,进行证候、药物频次、性味、归经、组方规律及新方等分析。结果:共纳入81篇文献、103首方剂,涉及药物131味;经分析可得出肝肾亏虚证、肝郁气滞证为最常见证型;药物多归肝经,药性以寒、温为主,药味多属甘味居多;药物多归补虚药类,使用频次位居前五的药依次为白芍、柴胡、甘草、川芎、茯苓、当归;最常用的药对为白芍-柴胡;并获得新的核心药物组合12组,新方5个。结论:帕金森病抑郁主要病位在肝,病属本虚标实,用药以补虚药为主;治疗上补泻兼施,注重调补肝脾肾三脏,以补益精血、疏肝解郁、健脾益气为主要治法,并为临床治疗帕金森病抑郁提供新方药。

Objective:Based on the TCM inheritance assistance platform(V2.5.0),to explore the medication pattern of depression in Parkinson's disease and to provide reference for the treatment of depression in Parkinson's disease by TCM.Methods:The literature on the use of Chinese medicine compound prescriptions for the treatment of depression in Parkinson's disease from the Chinese Journal Full Text Database(CNKI),Chinese Academic Journal Database(Wanfang Data),and Chinese Science and Technology Journal Full Text Database(VIP)from the establishment of the database to December 2022 were searched for syndrome,drug frequency,taste and nature,meridian entry,prescription pattern and new prescriptions.Results:A total of 81 papers,103 formulas,involving 131 drugs were included.The analysis showed that liver and kidney deficiency and liver depression and qi stagnation were the most common symptoms;the drugs mostly belonged to the liver meridian,and their properties were mainly cold and warm,and their flavors were mostly sweet;the drugs were mostly classified as tonic drugs,and the top five drugs used in order of frequency were Bai Shao,Chai Hu,Licorice,Chuanxiong,Poria,and Angelica;the most commonly used drug pair was Bai Shao-Chai Hu;and 12 groups of new core drug combinations and 5 new prescriptions were obtained.Conclusion:The main location of depression in Parkinson's disease is in the liver,and the disease is deficient in nature and real in symptoms,so the main medicine used is to tonify the deficiency;the treatment is both tonifying and diaphoretic,focusing on tonifying the three organs of liver,spleen and kidney,with tonifying essence and blood,relieving depression in the liver,and strengthening the spleen and benefiting qi as the main treatment,and providing new prescriptions for clinical treatment of depression in Parkinson's disease.
