血清C肽、自噬关键分子酵母Atg6同系物、纤维胶凝蛋白3在2型糖尿病肾病患者中的表达变化及其预测价值研究

Changes and predictive value of serum C peptide,ATG6 autophagy related 6 homolog and Ficolin-3 in type 2 diabetic nephropathy

目的:探讨血清C肽、自噬关键分子酵母Atg6同系物(Beclin1)、纤维胶凝蛋白3(Ficolin-3)在2型糖尿病肾病(DN)患者中的表达变化及其预测价值。方法:选取2型糖尿病(T2DM)患者135例,其中单纯T2DM患者73例(T2DM组),DN患者62例(DN组),并选取同期体检健康者60例为对照组。比较三组肾功能及血清C肽、Beclin1、Ficolin-3水平。分析肾功能与C肽、Beclin1、Ficolin-3的关系,以及血清C肽、Beclin1、Ficolin-3对DN的预测价值。结果:DN组Scr、BUN高于T2DM组和对照组,且T2DM组高于对照组(均P<0.05)。DN组eGFR低于T2DM组和对照组,且T2DM组低于对照组(均P<0.05)。DN组血清C肽水平高于T2DM组和对照组,且T2DM组高于对照组(均P<0.05)。DN组血清Beclin1、Ficolin-3水平低于T2DM组和对照组,且T2DM组低于对照组(均P<0.05)。血清C肽水平与Scr、BUN呈正相关,与eGFR呈负相关(均P<0.05)。血清Beclin1、Ficolin-3水平与Scr、BUN呈负相关,与eGFR呈正相关(均P<0.05)。血清C肽、Beclin1、Ficolin-3对DN均有一定的预测价值,三者联合的预测价值更高。结论:2型DN患者血清C肽水平升高,Beclin1、Ficolin-3水平降低,三者联合具有较高的预测价值。

Objective:To investigate the changes and predictive value of serum C peptide,ATG6 autophagy related 6 homolog(Beclin1)and Ficolin-3 in type 2 diabetic nephropathy(DN).Methods:A total of 135 patients with type 2 diabetes mellitus(T2DM)were selected,including 73 patients with T2DM alone(T2DM group),62 patients with DN(DN group),and 60 healthy subjects in the same period were selected as the control group.Renal function and serum levels of C-peptide,Beclin1 and Ficolin-3 were compared among the three groups.The relationship between renal function and C-peptide,Beclin1 and Ficolin-3,and the predictive value of serum C-peptide,Beclin1 and Ficolin-3 on DN were analyzed.Results:DN group had higher Scr and BUN than other groups,and T2DM group had higher Scr and BUN than control group(all P<0.05).The eGFR in DN group was lower than that in T2DM group and control group,and T2DM group was lower than control group(all P<0.05).The serum C peptide level of DN group was higher than that of T2DM group and control group,and T2DM group was higher than control group(all P<0.05).Serum Beclin1 and Ficolin-3 levels in DN group were lower than those in T2DM group and control group,and those in T2DM group were lower than those in control group(all P<0.05).Serum C peptide level was positively correlated with Scr and BUN,and negatively correlated with eGFR(all P<0.05).Serum Beclin1 and Ficolin-3 levels were negatively correlated with Scr and BUN,but positively correlated with eGFR(all P<0.05).Serum C peptide,Beclin1 and Ficolin-3 all had certain predictive value for DN,and the combined value of the three was higher.Conclusion:The level of serum C peptide increases in patients with type 2 DN,while the levels of Beclin1 and Ficolin-3 decrease,and the combination of the three has a higher predictive value.
