无充气经腋窝入路腔镜下甲状腺肿瘤切除术的临床效果

Clinical Effect of Pneumatic-Free Transaxillary Approach Laparoscopic Thyroid Tumor Resection

目的 探讨无充气经腋窝入路腔镜下甲状腺肿瘤切除术的临床效果。方法 选取盐城市第一人民医院2020年8月至2022年8月进行甲状腺肿瘤切除术的84例患者病例资料进行回顾性分析,根据手术方式不同,无充气甲状腺肿瘤切除术的患者41例为腔镜组,开放性甲状腺肿瘤切除术患者43例为开放组。对比两组手术相关指标、术后并发症以及患者对术后切口的满意度。结果相比较开放组,腔镜组患者术中出血量较多,手术时间较长,术后血钙较高,术后48 h引流量较多,住院时间较长,差异均有统计学意义(P <0.05);两组淋巴结清扫数目、术后甲状旁腺激素水平差异无统计学意义(P> 0.05)。两组术后并发症发生率差异无统计学意义(P> 0.05)。腔镜组术后切口满意度高于开放组(P <0.05)。结论 无充气经腋窝入路腔镜下甲状腺肿瘤切除相对开放手术不影响手术根治效果,且切口隐蔽,患者满意度高。

Objective To explore the clinical effect of inflation-free transaxillary approach laparoscopic thyroid tumor resection.Methods The case data of 84 patients who underwent thyroid tumor resection in the First People's Hospital of Yancheng City from August 2020 to August 2022were selected for retrospective analysis,and according to the surgical method is not allowed,41 cases of patients with inflation-free thyroid tumor resection were in the laparoscopic group,and 43 cases of patients with open thyroid tumor resection were in the open group.Surgery-related indexes,postoperative complications and patients' satisfaction with postoperative incision were compared between the two groups.Results Compared with the open group,patients in the laparoscopic group had more intraoperative bleeding,longer operation time,higher postoperative blood calcium,more drainage flow in the postoperative 48 h,and longer hospitalization time,and the differences were statistically significant(P<0.05);the differences in the number of lymph nodes cleared and the level of postoperative parathyroid hormone between the two groups were not statistically significant(P>0.05).The difference in the incidence of postoperative complications between the two groups was not statistically significant(P>0.05).Postoperative incision satisfaction was higher in the laparoscopic group than in the open group(P<0.05).Conclusions Inflatable transaxillary approach laparoscopic thyroid tumor resection relative to open surgery does not affect the surgical radical effect,and the incision is hidden with high patient satisfaction.
