"F"型置钉与FNS固定术对成人创伤性Pauwels Ⅲ型股骨颈骨折患者疗效、骨折愈合状况及髋关节功能的影响 被引量:1

Effects of"F"Screw Placement and FNS Fixation on the Efficacy,Fracture Healing and Hip Joint Function of Adult Patients with Traumatic Pauwels Type Ⅲ Femoral Neck Fracture

摘要 目的:探讨"F"型置钉与新型股骨颈内固定系统(FNS)固定术对成人创伤性Pauwels Ⅲ型股骨颈骨折疗效、骨折愈合状况及髋关节功能的影响。方法:选取2019年1月-2021年12月我院收治的78例成人创伤性Pauwels Ⅲ型股骨颈骨折患者作为研究对象,采用随机数字表法将患者分为对照组及研究组,各36例。对照组采用FNS进行治疗,观察组采用"F"型置钉内固定术进行治疗。观察并比较两组患者治疗疗效、术后情况、骨折愈合情况、髋关节功能、骨折复位质量及并发症发生情况。结果:观察组治疗总有效率80.56%高于对照组52.78%(P<0.05)。观察组住院时间、下地行走时间及完全负重时间均短于对照组(P<0.05)。观察组骨折愈合状况优于对照组(P<0.05)。两组患者Harris评分随时间发展而逐渐升高,且观察组术后1个月、术后3个月及术后6个月评分均高于对照组(P<0.05)。观察组骨折复位率77.78%高于对照组41.67%(P<0.05)。观察组并发症发生率低于对照组(P<0.05)。结论:"F"型置钉及FNS固定术均可以治疗成人创伤性Pauwels Ⅲ型股骨颈骨折患者,但"F"型置钉治疗疗效更好,并且力学稳定性能更好,术后并发症较少,利于患者术后恢复,值得在临床治疗中推广使用。 Objective:To investigate the effects of"F"screw placement and new femoral neck system(FNS)on the efficacy,fracture healing and hip joint function of adult traumatic Pauwels type Ⅲ femoral neck fracture.Methods:A total of 78 adult patients with traumatic Pauwels type Ⅲ femoral neck fracture admitted to our hospital from January 2019 to December 2021 were selected as the research objects.The patients were divided into control group and study group by random number table method,with 36 cases in each group.The control group was treated with FNS,and the observation group was treated with"F"type nail internal fixation.The curative effect,postoperative condition,fracture healing,hip function,fracture reduction quality and complications were observed and compared between the two groups.Results:The total effective rate of treatment in the observation group was 80.56%higher than that 52.78%in the control group(P<0.05).The hospitalization time,walking time and full weight-bearing time of the observation group were shorter than those of the control group(P<0.05).The fracture healing of the observation group was better than that of the control group(P<0.05).The Harris scores of the two groups increased gradually with time,and the scores of the observation group at 1 month,3 months and 6 months after surgery were higher than those of the control group(P<0.05).The fracture reduction rate of the observation group was 77.78%higher than that 41.67%of the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:Both"F"type screw placement and FNS fixation can be used to treat adult traumatic Pauwels type Ⅲ femoral neck fracture,but"F"type screw placement has better curative effect,better mechanical stability,less postoperative complications,and is conducive to postoperative recovery of patients,which is worth popularizing in clinical treatment.
