重度阻塞性睡眠呼吸暂停低通气综合征患者的睡眠结构障碍的相关性分析

摘要 目的:探讨重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的睡眠结构及呼出气一氧化氮(FeNO)与涎液化糖链抗原(KL-6)水平的相关性。方法:选取2022年6月至2023年6月江苏省南通市第二人民医院耳鼻喉科收治的OSAHS住院患者60例作为观察组,选取同期到我院健康体检者60例为对照组。比较2组患者睡眠结构及FeNO与KL-6的相关性。结果:2组总睡眠时间、睡眠潜伏期差异无统计学意义(P>0.05),但睡眠效率、快速眼动睡眠期(REM)睡眠、REM睡眠潜伏期优于对照组(P<0.05)。与对照组比较,观察组体质量指数(BMI)、呼吸暂停低通气指数(AHI)升高,最低血氧饱和度(LSaO_(2))降低(P<0.05)。观察组KL-6、FeNO水平高于对照组(P<0.05)。经Pearson相关系数分析,OSAHS患者FeNO与BMI、AHI、KL-6呈正相关,与LSaO_(2)呈负相关。结论:重度OSAHS患者的睡眠结构严重紊乱,FeNO、KL-6水平均明显升高,且呈正相关,能够用于临床评估OSAHS病情严重程度,在临床可推广应用。 Objective:To investigate the correlation between sleep structure and exhaled nitric oxide(FeNO) level and salivary liquefied glycoantigen(KL-6) in patients with severe obstructive sleep apnea hypopnea syndrome(OSAHS).Methods:Sixty patients with OSAHS in the Otolaryngology Department of the Second People′s Hospital of Nantong City,Jiangsu Province from June 2022 to June 2023 were selected as the observation group,and 60 healthy patients from our hospital during the same period were selected as the control group.The correlation of sleep structure with FeNO and KL-6 in 2 groups was compared.Results:The sleep indexes of the observation group were significantly different from those of the control group,and the sleep efficiency was significantly lower,and there was statistical significance between rapid eye movement sleep(REM) and REM sleep latency(P<0.05).Body mass index(BMI) and apnea hypopnea index(AHI) of the two groups were higher in the observation group than in the control group,but lower in the observation group of minimum blood oxygen saturation(LSaO_2),and the comparison between the two groups was statistically significant(P<0.05).Comparing the two groups of KL-6 and FeNO indexes,it was found that the observation group was higher,and the difference was significant(P<0.05).Pearson correlation analysis of indicators in the two groups proved that the FeNO index level in OSAHS patients was closely positively correlated with BMI,AHI and KL-6 indicators,and closely negatively correlated with LSaO_2.Conclusion:The sleep structure of patients with severe OSAHS is seriously disturbed,and FeNO,KL-6 levels are significantly increased with positive correlation,which can be used for clinical assessment of the severity of OSAHS and can be popularized in clinical application.
